2783009 Primary Facility 2018-09-25 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of the SWEPI Hepler 235/602 Well Pad in Sullivan Twp, Tioga County on 9/25/2018 at 1240 hours. This inspection was conducted in response to a Notice of Termination received by the DEP. The weather was cloudy, 61 degrees and 85% humidity.

The site has 7 producing wells and two highland production tanks. All planned PCSM BMPs appear to be installed and functioning properly. The site is well vegetated and stabilized. One small stretch of sock was observed off the northwestern corner of the pad (photo #4). After notifying the operator it was promptly removed (photo #8). All temporary E&S BMPs are now removed. A post-drilling well site restoration report has been accepted by the DEP.

The lease road is showing some minor erosional channels on the steeper portions.

This Notice if Termination is recommended for approval. No violations noted at this time.

I departed the site at 1310.
