2670530 Primary Facility 2017-12-13 Site Restoration No Violations Noted The Department of Environmental Protection (DEP) conducted a restoration inspection of the SWEPI LP - Wilson 286 well pad located in Charleston Twp, Tioga Co. on December 13, 2017 @1300 hours. The inspection was conducted in response to receipt of a Wellsite Restoration Report, via esubmission, for the 1H, 3H, and 5H. The weather was 21 degrees overcast, winds WNW 14 mph. Some snow cover on the ground, but vegetation still visible.

The pad size has been reduced to ~275' x 400' which SWEPI feels is necessary for the operator to safely perform all operations. The tank pad has been reclaimed. Both sediment traps have been converted into infiltration basins. The site is supporting >70% vegetative cover. The 7H well has been plugged. The 1H, 3H, and 5H appear shut in but are listed as "active".

The WSRRs will be reviewed and processed. No violations are noted at this time. I departed the site at 1500 hours.

2731985 Primary Facility 2018-05-18 Routine/Complete Inspection No Violations Noted During the inspection of this well pad the weather was sunny and the temperature was 66degrees Fahrenheit. No evidence of spills or releases were noted during this inspection. No erosion issues were noted during this inspection and the site is stabilized. No violations were noted during this inspection.

2826172 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Wilson 286 Well Pad in Charleston Twp, Tioga County on 1/14/2019 at 0925 hours. The inspection was conducted to assess site conditions. The weather was 12 degrees and sunny.

There are currently 4 well cellars on the pad. One is plugged and no production activities are occurring. There is no equipment being stored on the pad. The site remains stabilized and vegetated, no erosional issues observed today. The pony pad onsite has been reclaimed.

No issues or violations noted today, I departed the site at 0945.
