2450234 Primary Facility 2016-02-12 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Kinnan 845 site (ESX10-117-0204) located in Middlebury Twp., Tioga Co. on 2/12/2016 @ 1230 hours. The weather was 10 degrees and overcast with snow flurries. An adequate site ID sign was located at the entrance to the access road. The E&S plan and copies of the well permits were located in the mailbox. The receiving stream is listed as UNT Crooked Creek (WWF), UNT Losey Creek (WWF).

The Rock Lined Ditches (RLDs) along the access road appeared stable and functional. The access road also appeared to be in good shape. The site appeared mostly snow covered. The well pad measured ~400’ x 425’ with a smaller attached tank pad measuring ~100’ x 250’. The cut slope and fill slopes appear rocky and do not appear to have 70% vegetative cover. A single well head with adequate ID tag was visible onsite. No violations were noted during this inspection. Please contact me with any questions. I departed the site @ 1330 hours
