2798347 Primary Facility 2018-11-05 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of SWEPI's WOOD 513 WELL SITE in Rutland Twp, Tioga County on 11/5/2018 at 1120 hours. This inspection was completed in response to a Notice of Termination received by the department for this site's ESCGP. The weather was 47 degrees and raining.

The site has five producing wells and two production tanks. All PCSM practices were in place and appeared to be functioning properly. The site seems to be stabilized and no temporary E&S BMPs were observed. A well site restoration report has been accepted by the DEP.

This Notice of Termination is recommended for approval. No violations noted at this time.

I departed the site at 1150.
