2137440 Primary Facility 2013-02-08 Routine/Partial Inspection No Violations Noted An inspection was conducted at the Young 431 well pad in Shippen Township, Tioga County on January 31, 2013. The 1H well has been drilled and the 2H, 3H, 4H, 5H, 6H, 7H and 8H wells have been spudded. The pad surface was mostly snow covered; no puddles were evident on the pad. Water in the Rock Lined Ditches was screened for specific conductance; no elevated readings were found. The cut and fill slopes were uniformly vegetated and met the requirements of 25 Pa Code § 102.22(b) for Temporary Stabilization. No violations were cited during this inspection.
2220978 Primary Facility 2013-11-07 Routine/Partial Inspection No Violations Noted On Nov. 7, 2013 a routine partial inspection was conducted at the Houck 433 pad in Shippen Township, Tioga County. The last inspection conducted on behalf of the operator was on 11/5/2013. The 1H well has been drilled and 7 others have been spudded. The site is stabilize and the slopes are well vegetated. The E&S controls remain in place are are functioning and maintained. No violations were cited during this inspection.

2440910 Primary Facility 2016-01-14 Routine/Complete Inspection No Violations Noted The Department inspected the SWEPI LP site, the Houck 433 (ESX12-117-0019) located in Shippen Twp., Tioga Co on 1/14/2016 @ 1330 hours. The weather was overcast and 21 degrees with 1-1.5” snow on the ground. An adequate site ID sign was observed at the entrance to the access road. The old ESCGP was located in the box along with the drilling permits and onsite inspection log. The site was last inspected by company personnel on 1/11/2016 with “no visible issues” in the comments section. The current E&S plan onsite is the Rieppel Meter permit No. 5090804. The site was re-permitted in 2012 under the ESCGP#ESX12-117-0019. An updated copy of the permit and the E&S plan should be placed in the mailbox onsite.
2476425 Primary Facility 2016-04-19 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Houck L 433 site (ESX12-117-0019) located in Shippen Twp., Tioga Co. on 4/19/2016 @ 1130 hours. Present onsite for the inspection was Matt Bowman, OSR SWEPI. Mr. Bowman stated they had just completed the frack on the 26H well and had pulled up the containment Friday. SWEPI found three areas of elevated soil conductivity during their post-frack screening with readings between 5-6 mS/cm. SWEPI removed the material, excavating down ~1”, and back-filled the areas with clean material. Since no holes or tears in the containment were observed, Mr. Bowman stated the fluid most likely seeped over the edge and underneath the containment. A small area of standing water and mud was observed in the SW corner of the pad. The puddle appeared to be draining from a small pile of wet material nearby; the field conductivity was measured at ~2 mS/cm. A violation of Section 3218.2(a) OGA 58 Pa. 2523781 Primary Facility 2016-09-22 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Houck L 433 site (ESX12-117-0019) located in Shippen Twp., Tioga Co. on 9/22/2016 @ 1400 hours. The weather was sunny 75°, 49% humidity. A valid site ID sign was observed at the entrance to the access road. The onsite E&S log, E&S plan and a copy of the well permits were located in the mailbox. The site was last inspected by company personnel on 9/18/2016 with “very little sediment movement coming down hill in NW corner of the pad into ditch”, noted in the comments section.

The 26H well appears to be in the process of flowing back. The well is connected to a sand trap and flowing into an open top tank on containment. The fluid was observed in the secondary containment. The field conductivity was measured at 1200µS/cm. Another batter of tanks was located on containment in the SE corner of the well pad. Fluid was observed in this containment as well. The field conductivity of the fluid measured 230µS/cm.

2556012 Primary Facility 2017-01-27 Routine/Complete Inspection No Violations Noted The department of environmental protection inspected the SWEPI LP Houck 433 well site in shippen Township,Tioga County on January 27, 2017. The weather was 32° cloudy winds out of the west at 40 mph. Snow squalls observed in the area. The site was last inspected by company personnel on 1/22/2017. One well appeared to be in production on the well pad. Cut and fill slopes appear stabilized. Erosion and sediment control's appeared functional. No issues or violations were noted during this inspection. Please contact me with any questions.
2580864 Primary Facility 2017-04-10 Routine/Complete Inspection No Violations Noted The DEP inspected the SWEPI LP Houck 433 well site in Shippen Township Tioga County on 4/10/2017 at 1300 hrs. The weather was 73 and sunny. The flow back equipment has been removed from the site. No evidence of staining was observed where the tanks were located. E&S controls appear to be functioning. Standing water was observed in the sediment trap below the fill slope. Cut and fill slopes appeared stabilized. No standing water was observed on the pad surface. No violations are noted at this time. I departed the site at 1400 hours.
2660681 Primary Facility 2017-11-14 Routine/Complete Inspection No Violations Noted An RTNC inspection of the SWEPI LP Houck 433 well site in Shippen Twp., Tioga Co. was conducted on 11/14/2017 at 1350 hours. Personnel present was Mary Tassin, Shell OSR and myself. The top-hole rig was currently onsite. Miss Tassin stated they had just completed cement on the 6H well and would be onsite until possibly 11/26/2017, drilling the remaining wells.

All tanks containing fresh water and brine water were staged on secondary containment. Puddles on the pad surface near containment, were screened for field conductivity, were found to be within normal limits.

No violations are noted at this time. I departed the site at 1500 hours.

2708713 Primary Facility 2018-03-21 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the SWEPI LP – Houck 433 Well Pad located in Shippen Twp., Tioga Co. on 3/21/2018 @1245 hours. The weather was 38 degrees, partly cloudy with winds north @10mph. Brett McCoy, SWEPI OSR was present onsite for my inspection.

The Ensign 162 rig is currently onsite preparing to set the casing on the 6H well. The rig will begin drilling the 8H well on 3/23/2018. The rig has plans to drill 7 wells while onsite. The wells that were previously in production were shut-in and the production equipment was moved to make room for the rig. Two puddles were observed off the edge of containment with a field measured conductivity of <2 mS/cm. No pollution material was observed in the vicinity of the puddle; however tire tracks were clearly visible. The slightly elevated conductivity appears to be due to road salt. Mr. McCoy stated he would have a vac truck remove the puddles, as a precaution, immediately. No other issues were noted on the pad surfa

2724040 Primary Facility 2018-04-26 Routine/Complete Inspection No Violations Noted I met Charlie Young of SWEPI when I arrived. Ensign rig #162 on location, drilling at 4400' on the 2H well. Dry chemicals were near the edge of the sitewide containment, and I recommended to Charlie that the pallets be moved back from the berm. The cuttings backyard was very clean and well maintained. The containment berms on the pipe rack end of the rig were very flattened out from traffic driving over them; although no regulated substances were being stored on this end of the pad, I recommended to Charlie that he have the berms repaired.
2736172 Primary Facility 2018-05-31 Routine/Complete Inspection No Violations Noted The DEP inspected the SWEPI LP Houck 433 well site in Shippen Township Tioga County on 5/31/20108 at 1250hrs. The weather was 71 degrees, cloudy. Winds from the SSE, 11 mph. Humidity: 80%, Pressure: 29.98 in. The rig and containment have been removed from the site. 7 new wells were drilled, but appear to be shut-in. No tags were visible. No production equipment is onsite. No evidence of staining was observed where the tanks used to be located.

E&S controls appear to be functioning. Cut and fill slopes appeared stabilized. One puddle was observed on the pad surface with the field measured conductivity of 0.38 mS/cm.

No equipment was located on the tank pad. Slopes are well vegetated and silt sock is still in place. No violations are noted at this time. I departed the site at 1400 hours.

2789674 Primary Facility 2018-10-16 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Houck 433 well site in Shippen Twp, Tioga County on 10/16/2018 at 1000 hours. The weather was partly cloudy and 43 degrees. Ellie Niles, DEP WQS, was onsite with me at time of inspection. 2 wells are currently flowing back and another 7 are drilled but shut in. Personal from the pipeline appeared to be currently performing work.

No violations noted at this time. We departed the site at 1020.

2832618 Primary Facility 2019-01-28 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Houck 433 Well Site in Shippen Twp, Tioga County on 1/28/2018 at 1040 hours. The weather was 13 degrees, sunny and snow covered undisturbed areas of the pad. I spoke with Morgan Davis, Shell OSR, onsite and he accompanied me around site.

Keane Group was currently fracking the last stage of the 3H. Renegade Wireline Services was also onsite. Morgan stated that they were receiving water into frack tanks onsite from the above ground storage tank located east of the pad. The above ground storage tank (AST) onsite was receiving water from the Johnson impoundment through the fast-line. Trucks were also observed onsite offloading into the AST.

A small puddle within the frack tank containment on the west side of the pad (see photo #2) gave me an electrical conductivity reading of over range (>19.99ms/cm). Morgan said there was a vac truck onsite and that he would have the containment cleaned out momentarily. No puddles were observed outside of the contained area.

2853438 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted an INCIDENT inspection at the SWEPI LP - Houck 433 well pad located in Shippen Twp, Tioga Co. on 3/11/2019 @1130 hours. Matt Skolny, SWEPI LP was present on the pony pad for the spill investigation. Morgan Davis, SWEPI LP OSR was present on the main pad. The weather was 37 degrees, sunny, winds WNW 15mph, the previously deposited snow is melting causing the site to the very muddy with standing water.

April Heater, SWEPI LP reported a spill to the DEP's regional office on 3/8/2019 @1500 hours. Onsite personnel were breaking apart the "fastline", which was used to carry frack fluid from the AST on the pony pad to the main pad. Mr. Skolny stated they had run a number of courses of freshwater through the line to clean it out. When the line was dismantled the open end was placed in a tub, on secondary containment, on the NW corner of the pony pad. No fluid came out initially, but when the onsite personnel left to work on another area, fluid was discharged fr
