2852381 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Phillips 504 Well Site in Rutland Twp, Tioga County on 3/12/2019 at 1345 hours. The weather was partly cloudy and 30 degrees. The onsite inspection log was in the mailbox by the production tanks. Company personnel last inspected the site on 3/12/2019 noting no issues. The Act 9 sign was observed at the entrance and the mailbox contained the E&S Plan.

There are currently 3 wells that appear to be in production with another 3 that are inactive or currently shut in. The site is very well vegetated and stabilized. All PCSM BMPs are installed and functioning. There was no evidence of spills, releases, or accelerated erosion on the pad. No containment issues were observed.

No violations noted today, I departed the site at 1415.
