2487828 Primary Facility 2016-05-19 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP –Jones 276 site (ESX11-117-0043) on 5/19/2016 @ 1230 hours. The weather was 65 degrees partly sunny. The onsite inspection log stated the site was last inspected by company personnel on 5/17/2016 with “no changes” in the comments section.

One cellar was visible on the pad surface. The well was spudded 2/9/2012. No production equipment is located onsite. The pad surface measures ~500’ x 330’ with an adjoining tank pad ~200’ x 200’. All E&S controls appear to be functional at this time. No violations are noted at this time. I departed the site @1300 hours. 2570405 Primary Facility 2017-03-08 Routine/Complete Inspection No Violations Noted Inspection of SWEPI’s Jones D. 276 8H well in Jackson Twp., Tioga County conducted March 8, 2017 at 12:00 pm. The 8H is the only well on the pad. At the time of inspection; the 8H was almost completely submerged in the flooded cellar, no bubbling observed. I could see the top of the 9 5/8” well cap and two vent pipes that came up out of the water. I assumed the larger vent is the 9 5/8” vent and the smaller vent was the backside of the 9 5/8”, no way to tell with them being underwater, or if they were open on the head. I tested each vent with an Altair 5x gas meter, no gas present. The operator plans to “plug” this well in near future. The well is essentially already plugged at this point because only surface/intermediate csg is installed (cmt to surface) and after they cemented 9 5/8” intermediate casing (2067’, plug bumped, floats held) they never drilled out float equipment, leaving a 83’ cemented shoe track. They’ll “plug” this well by topping off with gravel from the float collar to surface to meet plug
2610382 Primary Facility 2017-07-05 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of SWEPI’s Jones D 276 8H well conducted July 5, 2017 at 10:30 am. The well has been plugged in adherence to ch. 78 regulations by filling cased hole from above casing seat to surface with gravel. At time of inspection, well was cut and buried below grade following plugging ops. No issues
2614777 Primary Facility 2017-07-13 Administrative/File Review No Violations Noted July 13, 2017: Certificate of well plugging has been received, reviewed, signed, and sent to file. This well has surface (and intermediate) casing installed with a cemented shoe track, never drilled out and completed. The operator topped cased surface hole with gravel, capped, and cut below grade to meet Ch. 78 plugging requirements. No issues.
