The 5H well was drilled. No puddles exhibiting elevated conductivity or erosion issues were observed at this time. All area were well vegetated and no issues were noted. No violations were cited during this inspection. - MRN
2798359 Primary Facility 2018-11-05 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of SWEPI's Scheible 898 Well Site in Deerfield Township, Tioga County on 11/5/2018 at 1310 hours. The inspection was conducted in response to a Notice of Termination received by the DEP. The weather was 47 degrees and cloudy.

One well listed as inactive is on this site. All PCSM practices appear to be in place and functioning properly. The site appears stabilized, well vegetated and no temporary E&S BMPs were observed. A well site restoration report has been accepted by the DEP.

I recommend this Notice of Termination for approval. No violations noted at this time.

I departed the site at 1330.
