Conductors have been set for the 4H - 6H wells. The 23H well is in production. The pad has not been restored, although is permanently stabilized. Some erosion observed at the southeast pad corner; requested that the erosion feature be repaired. Department received a request from SWEPI on 1/11/16 to extend the restoration requirements for the site. TLT

The Well Site Restoration Report (WSRR) was received on 11/16/16 and covers the 1V, 1H - 6H and 23H wells. Interim site restoration completed between July - August 2016. Post construction stormwater management (PCSM) BMP’s have been installed. Areas disturbed during the construction of the PCSM BMPs are permanently stabilized as per 25 Pa Code §102.22(a)(2) and temporary E&S controls have been removed. Recommend approval of the NOT. 2735122 Primary Facility 2018-05-29 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the SWEPI LP- Lopatofsky Unit 287 located in Charlestown Township, Tioga County on May 29, 2018 at 1400 hrs. the weather is sunny and 89°.

The NOT was terminated on 11/22/2016. The 1V, 1H - 3H and 23H wells have been drilled. The 1H-3H and the 23H appear to be in production. The tank and containment onsite inspection log was last updated May 28, 2018. No issues were noted. The access road and rock Lined ditches appear to be in good shape. Some minor erosion was observed at the top of the rock channel spanning from the center of the pad to the infiltration basin.

No violations were noted at this time. I departed the site at 1440 hrs.

2826372 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Lopatofsky 287 Well Pad in Charleston Twp, Tioga County on 1/14/2019 at 1120 hours. The inspection was conducted to assess site conditions. Weather: 21 degrees and sunny.

There are currently 4 wells onsite capable of producing with one Highland production tank. The site remains stabilized and no erosional issues were observed. PCSM BMPs remain in place and appear functional. All fluids on the pad surface were frozen solid today.

I departed the site at 1145 hours. No violations noted today.
