2178770 Primary Facility 2013-06-26 Routine/Complete Inspection No Violations Noted 3 gas wells producing.
2492832 Primary Facility 2016-06-14 Site Restoration No Violations Noted This inspection was conducted in response to a Notice of Termination received by the Department on June 13, 2016. No SWEPI representatives were present.

Also filed with the NOT were Well Site Restroation Reports for the four wells drilled at this location. A total of 1,026.08 tons of drill cuttings, 244.84 barrels of drilling fluid, and 1,829.85 barrels of fracing fluid were generated at the site. The waste generated was disposed of in approved landfill and treatment facilities.

The site was determined to meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was observed. All temporary erosion and sedimentation BMPs were removed. No violations were cited during this inspection.

Based on the conditions observed during this inspection, this ESCGP permit is recommended for termination, and the restoration reports are acknowledged.

2708451 Primary Facility 2018-03-22 Routine/Complete Inspection No Violations Noted No es log
2856350 Primary Facility 2019-03-18 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. Three wells are staged for production. The wells have well tags which say Shell Appalachia instead of Rockdale. The Department recommends replacing the well tags. No GPUs are present on site. Two large white plastic produced water tanks are present within steel containment with plastic lining. Less than one foot of freeboard exists within this containment. The Department recommends pumping out the containment so that at least two feet of freeboard exists within the containment. The produced water tanks are not appropriately labeled. The Department recommends labeling the produced water tanks. Infiltration basins are present at the southeast and northwest corners of the pad.

No E&S issues noted. No violations noted.
