2643408 Primary Facility 2017-10-03 Routine/Complete Inspection No Violations Noted On 10/3/17 at 1331 hrs I conducted a routine inspection at the Baker 128 well site. It was sunny and 73 degrees F. No one was present during my inspection.

The site was idle. The 128D well was drilled in 2008 and placed on inactive status on 2/17/14. Drilling activity on the 1H - 6H wells occurred between 2011 - 2012. Two additional wells, the 21H and 23H, were permitted in 2014, but were never drilled and the permits have expired.

An ESCGP-2 (ESX10-117-0066(01) was issued on 9/17/15 and replaces the original ESCGP-1, which authorized construction of the site. SWEPI obtained a restoration extension on 3/17/16. Since then, interim restoration of the site has been completed. The tank storage pad, formerly east of the well pad, has been restored. Infiltration Basin 1 has been constructed near the site entrance and Infiltration Basin 2 has been built in the former location of Sediment Traps 1 & 2. 2652946 Primary Facility 2017-10-26 Site Restoration No Violations Noted A restoration inspection for the SWEPI LP Baker 128 ESX10-117-0066(01) was completed on October 26, 2017 at 1430 hours. The weather was 50 degrees, partly cloudy. Due to conditions observed on site during the inspection this NOT is recommended for approval.

I departed the site at 1545 hours.

2829319 Primary Facility 2019-01-18 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Baker 128 Well Pad in Delmar Twp, Tioga County on 1/18/2019 at 1050 hours. The pad had a 1/4 inch of snow cover and all fluids on the pad surface were frozen.

No wells onsite are currently capable of production. No production equipment was observed onsite. The reclaimed pony pad to the northeast is well vegetated. The rest of the pad is also well vegetated and stabilized. No erosional issues observed.

No violations noted today, I departed the site at 1105 hours.
