The 5H and 6H wells were spud on 12/8/11 (80' of conductor pipe installed), however never drilled. A stained area was noted on pad surface. Staining was grayish-brown in color. SWEPI stated that cellar water and surface water which had accumulated in the conductor pipe was discharged to pad surface to infiltrate. Staining on pad was surficial. Requested that SWEPI evaluate stained area and determine whether remedial actions are necessary. TLT

The 1V well was placed on inactive status on 5/9/14. SWEPI's two-year restoration extension request was approved on 5/4/16. A new ESCGP2 application is currently under review. 2779459 Primary Facility 2018-09-25 Routine/Complete Inspection No Violations Noted Routine (pre-plugging) inspection of 4 wells on the pad which are "top-holed"; surface/intermediate casings installed and cemented, cemented shoe track in 9 5/8" casing, and no openhole below the casing shoe. It was reported no oil or gas zones were encountered. At the time of inspection, all four wells had cellars full of gravel. Wells are capped. Annular vents were present. No gas was detected at the vents or in the gravel next to well caps. Also on location is the recently plugged 1V well which is cut and buried. There were no open holes, discharges, venting, or any other obvious issues observed on location during this inspection.

2815363 Primary Facility 2018-12-13 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection on the Burke pad was conducted December 13, 2018. I spoke with the plugging contractor on location, who is in the process of plugging the four remaining wells on the pad; the 1H, 2H, 3H, and 4H. All four wells are "topholed"; surface/intermediate casings were set and cemented upon drilling, the 9 5/8" has a ~80' cemented shoe track below float valve, and no open hole below the 9 5/8" casing shoe. The contractor is onsite to fill the inside of the casing to surface with gravel to satisfy the regulatory requirements of plugging a non-coal well. No obvious issues or concerns were observed at this time.
2819277 Primary Facility 2018-12-26 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (post-plugging) inspection of four vertical uncompleted wells was conducted December 26, 2018. All equipment and personnel are off location following the final plug and abandonment of the 1H, 2H, 3H, and 4H. The wells are now cut below grade, marked, and buried. No gas was detected in the gravel above the buried well locations. No open holes, discharges, venting, or other obvious issues were observed at this time.
