2029646 Primary Facility 2011-12-27 Incident- Response to Accident or Event No Violations Noted On 12/27/11 John Watson, the Health Safety and Environmental technician, explained that during a recirculation of the drilling mud, (bottom up), the electrical generators stalled out and they lost power. The drilling mud flowed to the now non-functional shakers and filled the hopper and over-flowed. Initially, only a small amount of spilled mud was observed, but the drilling supervisor checked recirculation quantity and they found more spilled material around the mats. I conducted a walk around of the area within containment as well as the perimeter of the containment. No mud had overtopped the containment. A vacuum truck was used to recover the mud. Wood pellets were used to recover some additional mud. John Watson has indicated to me that approximately 50 barrells of drilling mud was spilled, 45 barrells was recoved with the vac truck and some more with the wood pellets. It was agreed that this material will be recovered when the mats are removed after drilling has been completed. - MRN

2686563 Primary Facility 2018-01-25 Routine/Complete Inspection No Violations Noted I inspected the SWEPI LP Nestor 551 well pad located in Delmar Township, Tioga County on January 25, 2018. The weather was 20°, partly cloudy winds W at 7 mph. Some ice was observed on the pad surface.

For wells were observed drilled in production on the pad, the 1H, 2H, 3H, and 6H. Well tags were observed.

No violations or issues are noted at this time. I departed the site at 1430 hours.

2772888 Primary Facility 2018-08-27 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of the SWEPI LP - Nestor 551 well site located in Delmar Twp, Tioga Co. on 8/27/2018 1330 hours. The weather was 78 degrees and sunny. Present for the inspection were myself and Korey Starkey, DEP Env. Trainee.

The access road, well pad, and surrounding controls were observed during the inspection. The pony pad has been removed and the area restored. The well pad has been reduced to ~275'x400'. The reclaimed areas appear stabilized by 70% perennial vegetative cover. No erosion or sedimentation was observed. No temporary BMPs were observed onsite.

Due to conditions observed onsite during the course of this inspection, this NOT is recommended for approval.

No violations are noted at this time. We departed the site at 1400 hours.
