Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2009851 Primary Facility 2011-10-11 Routine/Complete Inspection No Violations Noted access road sediment runoff into state forest. viols charged to 13H
2068857 Primary Facility 2012-05-16 Follow-up Inspection No Violations Noted closing E&S viols. fracking 6 gas wells
2073211 Primary Facility 2012-06-01 Compliance Evaluation Viol(s) Noted & Immediately Corrected 10 brl frack fluid release from valve under pressure
2253287 Primary Facility 2014-03-11 Incident- Response to Accident or Event No Violations Noted Production liquid leaked out behind separators when flange developed hole. Violations cited to ESCGP.
2312587 Primary Facility 2014-10-14 Follow-up Inspection No Violations Noted Clossing violations from 11 MAR 2014, when production liquid leaked out of flow line.
2592328 Primary Facility 2017-05-03 Routine/Partial Inspection No Violations Noted This is an inspection of DCNR 595 Pad E.

Electrical conductivity readings w an Oakton PCSTestr 35: production tank containment--0.13 mS/cm, separator containment--0.15 mS/cm, pad--1.2 mS/cm. Onsite during this inspection were 2 Seneca workers tending to the separators. The basin down gradient of the pad (photo) is not on the plan drawing. The basin along the access road (photo) is listed as a "rain garden" on the plan. Time: ~1500-1555 hrs

2820629 Primary Facility 2018-12-28 Site Restoration No Violations Noted This inspection covers DCNR 595 Pad E (not Pads C & G).

NOT denied. Slopes around the pad are temporarily stabilized with hydroseed and sparse vegetation (photo 2). Filter sock (photo 1) is considered a temporary E & S control and hydro seed temporary stabilization; ESCGP termination requires "permanent stabilization", and removal of temporary E & S controls. I took electrical conductivity readings from pooled water on site with an Oakton PCSTestr 35. A puddle near the production tanks returned 0.17 mS/cm. Air temp: 45F JLP
