2153951 Primary Facility 2013-03-26 Routine/Partial Inspection No Violations Noted A routine partial inspection of the Baker 1105 pad in Deerfield Township, Tioga County was conducted on March 26, 2013. At the time of this inspection the pad was partially snow-covered; no erosion issues or puddles exhibiting high conductance were observed. Three permits were issued on 2/11/2011 for the 1H, 2H and 3H wells. The wells were spudded on 3/4/2011 (1H), 3/6/2011 (3H) and 3/8/2011 (2H), no record of any drilling activity has occurred since those dates.Violations were cited against the 1 H well for failure to restore, no E&S plan on site, no well permits on-site and no site ID sign posted.
2159424 Primary Facility 2013-04-18 Routine/Partial Inspection No Violations Noted On April 18, 2013 a follow-up inspection was conducted at the Baker 1105 well pad in Deerfield Township, Tioga County to determine if violations cited during a March 26, 2013 inspection (2153922) have been addressed.

The operator has submitted a Notice of Intent to plug all 3 wells on the (conductors only). At the time of inspection, the top of all 3 conductors were covered with plates. The cellars were full of gravel. There were mouseholes set for each well. The operator plans to plug next week and restore the site thereafter. No obvious issues were observed.
2715444 Primary Facility 2018-04-06 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection on the Baker pad was conducted April 6, 2018. At the time of inspection, all three wells on the pad (conductors only) have been plugged as of yesterday. The plugged wells are cut below grade and buried, and site restoration is underway. No obvious issues were observed during this inspection.
