2337126 Primary Facility 2015-01-20 Site Restoration No Violations Noted Well Site Restoration Reports and ESCGP Notice of Termination (NOT) received 12/24/14. Restoration Reports acknowledged 1/21/15. Recommend denial of Notice of Termination due to snow cover and inability to verify permanent stabilization. NOT should be resubmitted when ground conditions improve. TLT
2392800 Primary Facility 2015-07-07 Site Restoration No Violations Noted The Department inspected the SWEPI LP site – Wilson 283, ESX10-117-0257(1), Charleston Twp., Tioga CO. on 7/7/15 @ 1230 hours. The weather was 80 degrees and partly sunny. This inspection was conducted in response to a Notice of Termination received by the Department on 7/1/2015. It is unclear whether installed PCSM BMP’s are adequately addressing runoff from the site therefore, a technical review from the Oil and Gas District Technical Services Section has been requested.
2398164 Primary Facility 2015-08-11 Routine/Complete Inspection No Violations Noted
2439060 Primary Facility 2015-12-30 Site Restoration No Violations Noted The Department inspected the SWEPI Wilson 283 site (ESX10-117-0257(1), Charleston Twp., Tioga Co. on 12/30/2015 @ 1130 hours. The site was previously inspected on 7/7/2015 in response to receipt of a Notice of Termination. A technical review was requested due to concerns regarding adequate PCSM BMPs along the access road. On 8/27/2015 an onsite technical review was conducted by myself and Jeremy Rusczyk. SWEPI was directed to install additional waterbars along the access road, and extend the roadside channel along the east side of the access road. The NOT was returned.

2812637 Primary Facility 2018-12-10 Routine/Complete Inspection No Violations Noted Cold/windy at time of inspection.

Well tags are not legible. (To much paint/grinding done to well tag). Recommend replacement well tags or remove paint from existing well tags.

2826249 Primary Facility 2019-01-14 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Wilson 283 Well Pad in Charleston Twp, Tioga County on 1/14/2019 at 1010 hours. The inspection was conducted to assess site conditions. The weather was 17 degrees and sunny.

There are currently 4 wells capable of production with 1 highland production tank onsite. This pad has been reduced in size and very little pad surface is not vegetated. The site remains stabilized and well vegetated. There are some minor erosion channels in the access road. All fluids on the pad surface were frozen solid today.

No violations noted today, I departed the site at 1035.

2839266 Primary Facility 2019-02-13 Routine/Complete Inspection No Violations Noted Cold snowy/windy at time of inspection.

Well tags have been cleaned/replaced since last inspection.
