2195777 Primary Facility 2013-08-15 Routine/Partial Inspection No Violations Noted On August 15, 2013 a routine partial inspection was conducted at the Stanley 1106 pad in Osceola Township, Tioga County. The last inspection conducted on behalf of the operator was on 8/13/2013: the only issue noted was that the cellars are holding water. There are seven drill permits issued for this site, the 1V and 2H wells have been drilled, the well tags installed and the permits are active. The status of the 1H, 3H, 4H, 5H and 6H well permits are listed as Operator Reported Not Drilled. The ESCGP is active. The site is well vegetated and stabilized. No puddles were observed on the pad surface and no issues were noted. No violations were cited during this inspection.

2446947 Primary Facility 2016-02-02 Incident- Response to Accident or Event No Violations Noted The Department inspected the SWEPI LP - Stanley 1106 (ESX10-117-0243) located in Osceola Twp., Tioga Co. in response to notification of a spill by Patrick Bernethy, Environmental/Regulatory Tech from Shell Appalachia. Mr. Bernethy contact the WSQ @ 1150 hours on 1/26/2016 to notify the Department that SWEPI LP removed the temporary containment liner used during well completion activities on the 23 H well (API#117-21792) at the Stanley 1106 pad. As part of SWEPI LP’s standard procedures, soil conductivity readings were collected from the pad following the liner removal. Soil conductivity measurement points exhibited readings above background in the area of the temporary liner; approximately 6-8 mS. SWEPI LP was not notified during completion activities of any spills by the completions contractor, US Well Services.
2580713 Primary Facility 2017-04-10 Routine/Complete Inspection No Violations Noted The DEP inspected the SWEPI LP Stanley 1106 well site on 4/10/2017 @1000 hours. The weather was 60 degrees and sunny. A restoration of the tank pad and installation of post-construction Stormwater management BMPs was ongoing at the time of the inspection. Perimeter BMPs appeared to be installed around the earth disturbance area. Two wells were observed in production on the main pad. No fluid was observed in secondary containment surrounding the baker tanks. No puddles were observed onsite. No violation are noted at this time. I departed the site @1100 hours.
