2835360 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Shaw Trust 500 well pad in Sullivan Twp, Tioga County on 2/14/2019 at 1510 hours. The weather was sunny and 59 degrees. The site ID Sign was observed at the site entrance. The E&S Plan and inspection log was found in the mailbox by the gate of the access road. The last inspection was conducted today and noted no issues.

There are currently 6 wells producing onsite, the 1H - 6H. The site remains well vegetated and stabilized. Only erosion was a small channel in the access road. Containment structures were in good shape. All field measured fluids on the pad surface returned normal conductivity readings. PCSM BMPs were observed as functional.

No violations noted today. I departed the site at 1545.
