Tioga Co. conducted October 26, 2016 at 12:15 pm. At the time of inspection; the cellar was filled with water with 3” tagged standpipe monument about 8’ high. The top of the monument was open ended “T”, ran hose for Altair 5x gas meter in the vent; no gas detected. There were (7) active gas wells in production on the pad; #590885 (117-20966), #590884 (117-20975), #590883 (117-20974), #590892 (117-20985), #590890 (117-20983), #590889 (117-20982), #590888 (117-20981). Conducted visual inspection of WH and cellar; no obvious conecrns.
2535686 Primary Facility 2016-11-03 Site Restoration No Violations Noted Well Site Restoration Reports (WSRRs) received on 11/1/16. Since the new 25 Pa Code Ch. 78a regulations are in effect, WSRR's are to be submitted on the updated form through the Greenport website. This information was conveyed to EQT along with the request that the WSRRs be submitted electronically on the updated form. Interim restoration has been completed at the site and vegetative cover is becoming established. TLT
2544170 Primary Facility 2016-12-06 Site Restoration No Violations Noted Restoration inspection. Notice of Termination (NOT) received on 11/16/16 and Well Site Restoration Report submitted through Greenport on 12/6/16. Post-construction stormwater management BMPs have been installed. Vegetation is becoming established, however, entire site has not achieved 70% uniform vegetative cover and silt sock remains in place at the former Impoundment #1 location. Recommend that NOT be denied. Recommend that the NOT and WSRR be resubmitted after vegetation becomes more established and silt sock is removed. TLT
2596526 Primary Facility 2017-05-19 Site Restoration No Violations Noted On 5/19/17 at 1402 hrs I conducted an inspection of the Phoenix Pad R well site. No one was present during the inspection. It was mostly sunny and 84°F.

I observed the site and reviewed the erosion and sedimentation (E&S) routine inspection log, the Notice of Termination (NOT) and the Well Site Restoration Report (WSRR).

The well site has been restored and post-construction stormwater management (PCSM) BMPs have been installed. Temporary E&S controls have been removed and the project is permanently stabilized as per 25 Pa Code Section 102.22(a)(2). The last routine E&S inspection was conducted on 5/15/17.

Based on existing conditions, I recommend approval of the NOT. The WSRR was approved on 5/22/17. No violations were cited during this inspection. I concluded the inspection at 1433 hrs. TLT

2702163 Primary Facility 2018-03-06 Routine/Partial Inspection No Violations Noted The department of environmental protection conducted an inspection of the EQT Phoenix pad R in response to report of a spill to the ground surface received March 3, 2018 @1315 hours. Present onsite was Jim Kelley, EQT Assistant Superintendent.

A stuck dump valve caused an overfill of a 100 BBL capacity storage tank. EQT estimated the spill of produced water to be between 30-40 gallons. The fluid settled in a low area on the pad surface adjacent to the tank. The impacted area, approximately 15' x 5' x 10", was excavated and "burrito" wrapped in plastic until a waste rolloff container was delivered.

As of today's inspection, the impacted material has been moved to the rolloff container. The excavated area has been backfilled with clean gravel. Mr. Kelley reported the malfunctioning valve has been repaired. A copy of the monthly tank inspection report, required under 25 Pa Code 78a.57(i), was requested from the Operator.

I departed the site at 1400 hours.

2807939 Primary Facility 2018-11-28 Routine/Partial Inspection No Violations Noted The DEP conducted an INCIDENT inspection of the EQT Pad R site located in Duncan Twp, Tioga Co. on 10/28/2018 at 1015 hours in response to spill notification #338402.

Cory Guiles of EQT reported that at 2:40 pm on 11/27/2018, "there was a release of hydraulic fluid to the pad surface. There was a mechanical failure on a water truck that allowed 20 gallons of hydraulic fluid to be released to the pad surface of Phoenix R. They are in the process of cleaning up the impacted soil now and will store it in drums."

As of today's inspection, no personnel were located onsite. I contacted Corey Giles, EQT to confirm the location of the spill. He stated he was under the impression it was in front of the production tanks, approximately 1 barrel of material was removed and had already been backfilled with clean material. During the course of the inspection Tim Ryan, EQT well tender appeared onsite and showed me the location of the spill. A small area adjacent to the well heads appears to have been recently disturbed.
