Tioga County conducted August 16, 2016 at 2:00 pm. The operator has sent Notice of Intent to Plug this well along with planned procedure for review. At the time of inspection; the 1V was the only well on the pad, Shut-in with lower master valve & nightcap installed atop production csg. spool w/tubing hanger. There was no pressure gauge on the cap for production csg. pressure. The production csg. wing valve was set up with a Taylor plug valve for crystal gauge input, no visible gauge. The cellar was filled with gravel with one vent pipe plumbed to surface for the backside of 4 ½” production casing. No significant amount of gas was detected with Altair 5x gas meter at the vent, and no real measureable flow. The 4 ½” is cemented to surface, so it’ll be left in the hole during plugging. The operator’s planned procedure calls for CIBP (5500’) to be set in 4 ½” above top perfs (Marcellus), dump bail cement on top, set plug (2, 100’) 4770’-4670’ and plug

Tioga County conducted August 23, 2016 at 2:00 pm. The operator has sent Notice of Intent to Plug this well along with planned procedure for review. At the time of inspection; the 1V was the only well on the pad, Shut-in with lower master valve & nightcap installed atop production csg. spool w/tubing hanger. Operator submitted planned plug date for August 21. No one was on location. Operator has contacted the Department saying they’re behind schedule and will confirm a later date. No concerns were noted during the inspection, No Violations.
2518353 Primary Facility 2016-09-12 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of SWEPI’s Dietz 490 1V gas well in Richmond Twp. Tioga County conducted September 12, 2016 at approximately 11 am. At the time of inspection; workover rig was rigged up on the 1V for plugging ops. I met with the Companyman to discuss operations on the pad. There was no pressure on the well when they arrived, indicating bottom RBP set above top perfs held since the well was temporarily P&A’d. Top RBP could not be latched left in hole @ 5600’, The rig reentered, set CIBP at 5500’. Approximately 9 gallons of cement was spotted with a dump bailer on top of the bridge plug in the 4 ½”. The Comanyman is now waiting on R&R Cementers who will mix and pump through workstring the remaining (3) balanced plugs at (4770’-4670), (1119’-919), and (421’-221’). The crew plans to WOC overnight, top off 4 ½” csg with gravel, cut casing below ground level with appropriate markings and pull off location. No Violations noted during this inspection.
2522788 Primary Facility 2016-09-26 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of SWEPI’s Dietz 490 1V gas well in Richmond Twp. Tioga County conducted September 26, 2016 at 1:20 pm. The well was plugged approximately 2 weeks ago. At the time of inspection; the well monument was buried in the cellar and covered with gravel. I was able to tag the cellar and monument with a metal detector. The well is buried at these exact coordinates; N 41.78297, W -77.03059. I used a bar to make ½”diameter x 1’ hole above the well to probe the ground with a rod and hose hooked up to Altair 5x gas meter. I field reading taken with an Altair 5x gas meter yielded no gas in any amount while monitoring for approximately 10 minutes.
2631652 Primary Facility 2017-08-31 Site Restoration No Violations Noted SWEPI did final restoration at this site. The only remnants of the site are a rock entrance to the field (photo), and a piece of filter sock that SWEPI left for the landowner.
