2127326 Primary Facility 2013-01-03 Routine/Partial Inspection No Violations Noted A routine partial inspection of the Hudson 575 pad in Charleston Township was conducted on January 3, 2013. The well pad was snow-covered and no erosion issues were evident. The 1V well has been drilled. The last inspection conducted on behalf of SWEPI was 12/18/12. 25 PA Code §102.4(b)5(x) requires inspections of BMPs on a weekly basis. The access road was covered with over 12 inches of snow and was inaccessible to vehicular traffic. This should be addressed. No violations were cited during this inspection.
2224640 Primary Facility 2013-11-22 Routine/Complete Inspection No Violations Noted
2588280 Primary Facility 2017-04-27 Routine/Complete Inspection No Violations Noted On 4/27/17 at 1516 hrs I conducted a routine inspection of the Hudson 575 well site. It was mostly cloudy and 69 degrees F. No one was present during the inspection.

The site, which consists of the access road and a single-well pad, was idle. The 1V well has been drilled and was placed on inactive status on 5/29/14. The topsoil stockpile is permanently stabilized. Vegetative cover on the pad slopes was somewhat sparse and areas not vegetated were rocky.

An ESCGP-2 was issued on 10/1/15; this replaces the original ESCGP-1, issued on 6/29/10, which authorized construction of the existing well pad. SWEPI requested a two-year extension to the restoration requirements on 9/29/15. SWEPI recently provided an update to their restoration schedule; they tentatively plan to complete interim restoration of the site during the second quarter of 2017.

The last routine E&S inspection was conducted on 4/26/17. The original E&S plan was maintained in the mailbox, however, this should be replaced with the new ESCGP-2 E&

2765727 Primary Facility 2018-07-30 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of the SWEPI LP-Hudson 575 located in Charleston Twp., Tioga Co. on 7/30/2018 @1400 hours. The weather was 76 degrees and raining.

The site consists of 1020' of access road and a small 150'x200' well pad. The site appears stabilized by vegetation. The landowner is storing farm equipment on the pad surface. Remnants of manure and agricultural lime stockpiles were observed on the pad.

The WSRR for API#117-20864 has been reviewed and processed.

Due to conditions observed during this inspection, this NOT is recommended for approval. No violations are noted at this time. I departed the site at 1430 hours.
