2558792 Primary Facility 2017-02-02 Routine/Partial Inspection No Violations Noted Routine partial inspection. Site was idle and snow covered. The 2H, 5H and 6H wells are in production and the 1V well was placed on inactive status on 5/29/14. The site has not yet been restored. SWEPI submitted a 2-year restoration extension request on 12/10/14; the request was approved on 6/18/15. No violations were cited during this inspection. TLT
2619015 Primary Facility 2017-07-25 Site Restoration No Violations Noted On 7/25/17 at 0836 hrs I conducted an inspection at the Buckwalter 429 well site. No SWEPI representatives were present during the inspection. It was overcast and 67 degrees F.

I observed the site and reviewed the erosion and sedimentation (E&S) routine inspection log and the Well Site Restoration Report (WSRR).

The well site has been restored and post-construction stormwater management (PCSM) BMPs have been installed. Temporary E&S controls have been removed and the project is permanently stabilized as per 25 Pa Code Section 102.22(a)(2). The last routine E&S inspection was conducted on 7/23/17.

A 500 barrel (bbl) tank was staged in secondary containment. Standing water was observed in the secondary containment; the specific conductance field measurement was 485 micro Siemens/cm. Puddles on the pad were also tested; readings ranged between 232 - 258 micro Siemens/cm.

The WSRR was acknowledged on 7/25/17. No violations were cited during this inspection. I concluded the inspection at 0942 hrs. TLT

2632247 Primary Facility 2017-09-05 Administrative/File Review No Violations Noted On 9/5/17 I reviewed and acknowledged the Buckwalter 429 Well Site Restoration Report (WSRR). The report was electronically submitted on 7/23/17. Site conditions were verified during an ESCGP Notice of Termination inspection conducted on 7/25/17; the associated inspection reports document that post-construction stormwater management practices are installed and the site is permanently stabilized.

No violations are cited in association with this administrative inspection. TLT
