The site was stabilized, all E&S controls remain in place and appear to be functional. The 5H well (117-20856) was drilled from 11/20/10 to 1/8/11, but no well tag was observed. Cellars for the 1H (117-21365) and 2H (117-21366) have been set, but the wells have been reported not drilled by the operator. All permitted wells on this pad have been drilled, therefore the pad must be restored and well site restoration reports should be submitted. No violations were cited during this inspection.

A large gate at the site entrance was locked. The Department has not been provided a key to access Seneca Resources’ sites. An attempt to contact the operator was unsuccessful due to lack of cell service.

Failure to provide free and unrestricted access constitutes a violation of Section 3258(b) of 58 Pa.C.S. §§ 3201-3274 (relating to Development) (“2012 Oil and Gas Act”) which requires the owner, operator or other person in charge of a property, facility, operation or activity under this chapter, upon presentation of proper identification and purpose either for inspection or to remediate or otherwise respond to a well control emergency, by agents or employees of the department, shall provide free and unrestricted entry and access. I departed the site @1130 hours.

2480317 Primary Facility 2016-04-26 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the Seneca Resources – DCNR 007 Pad G well site (ESX10-117-0130 (01) on 4/26/2016 @1230 hours. The weather was 60° and cloudy with periods of rain showers. The winds were WSW @ 16mph and 69% humidity. The onsite E&S log for the DCNR 007 Pad G and Vert.50259 states the site was last inspected by company personnel on 4/18/2016. The boxes under site status for Production, Final Reclaimed, Signage, E&S plan, and E&S BMPs were initialed.

Seneca has placed a lock labeled “DEP” on the gate at the entrance to the site. Keys were provided to the Department to access the site. The violation of 3258(b) of 58 Pa.C.S §§3201-3274 (relating to Development) (“2012 Oil and Gas Act”) issued 4/8/2016 for not provided free and unrestricted access is considered resolved at this time. As of today’s inspection one well (API#117-20856) 5H was observed on the pad. A small poly tank appears to be connected to an annulus vent. Two constructed cellars were also observed. Two

2598916 Primary Facility 2017-05-18 Routine/Complete Inspection Violation(s) Noted On 5/18/17 at 1443 hrs I conducted a routine inspection of the DCNR 007 Pad G well pad. It was mostly sunny and 87 degrees F. No one was present during my inspection.

One well, the 5H, has been drilled but is not in production; drilling of the 5H was completed on 1/8/11. Two well permits, the 1H and 2H, were issued in 2011; Seneca reported the wells as not drilled and the permits expired on 11/2/12. No new permits have been issued for the site since 2011. It does not appear that the well site has been restored and the pad remains at full size (approximately 300 feet x 600 feet). Failure to restore the well site is a violation of 58 Pa C.S. Section 3216(a) and 25 Pa Code Section 78a.65 (a)(1).

On 2/29/16, Seneca submitted a Well Site Assessment Summary to the Department. The Summary milestones for Pad G included submittal of Well Site Restoration Reports (WSRRs) and the Notice of Termination (NOT) for the ESCGP. The well has undergone plugging operations at the end of last year but still holds an active status because a certificate of plugging was never received. The purpose of this inspection was to check the status of the well. At the time of inspection, the well has a blanked spool installed with a needle valve on the cap. The cellar was filled with gravel. There were 3 annular vents coming out of the gravel-filled cellar, all of which were shut in with bull plugs. There was no plugged well marker or monument on the well. It was unclear at the time of inspection what the operator's intentions are with this well.

The well has undergone plugging operations last fall and the operator has recently submitted a certificate of well plugging for the well. At the time of inspection, the cellar is full of gravel. There was a new casing spool and a night cap above ground level, and three inaccessible annular vents extending above ground level. At a minimum the well does not appear to meet the plugged well marking requirements outline in 25 PA Code Ch. 78.96. Please contact me at your convenience.
