2019085 Primary Facility 2011-11-09 Routine/Complete Inspection No Violations Noted
2507815 Primary Facility 2016-08-03 Administrative/File Review No Violations Noted The Heath 418 Well Site was inspected in response to a Notice of Termination (NOT) and Well Site Restoration Report (WSRR) received by the Department on June 29, 2016.

The well site has been partially restored. Areas not needed for production have been decompacted and vegetated. A total of 332 tons of drill cuttings were generated at the site and diposed of at approved landfill and recycling facilities.

The site was determined to not meet the permanent stabilization requirements of 25 Pa. Code § 102.22(a), and the NOT was returned (Inspection Report No. 2505512). While the waste reporting for the site appears accurate, the lack of permanent stabilization prevents the WSRR from being approved until the NOT is re-submitted and accepted.
