2752979 Primary Facility 2018-06-04 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was not producing. This inspection was conducted as part of a pressure build up test. The 9x13 and the 13x20 annulus vents were shut-in on Friday 6-1-18 when the Transducers were installed. The 5x9 annulus vent was connected and venting to a poly tank on location. The 5x9 annulus vent for the other wells on this pad were also connected to this poly tank. The tank was tested with a Altair 5x multi gas meter calibrated on 6-4-18 for combustable gas with the results being 25% LEL( lower explosion limit) and 0% Methane/ by volume at the time of this inspection. 0 psi( pounds per square inch) was observed on the gauge for the 13x20 annulus vent , 0 psi( pounds per square inch) was observed on the gauge for the 9x13 annulus vent and 0 psi( pounds per square inch ) was observed on the 5x9

annulus vent. No violations noted at the time of this inspection.

2753370 Primary Facility 2018-06-07 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was not producing. This inspection was conducted as part of a pressure build up test that began on 6-4-18.The 5x9 annulus vent was connected to a poly tank on location. The 5x9 annulus vent for the other wells on this pad were also connected to this poly tank. The tank was tested with a Altair 5x multi gas meter calibrated on 6-7-18 for combustable gas with the results being 0 LEL(lower explosion limit) and 0 Methane.1 pounds per square inch) was observed on the gauge for the 13x20 annulus vent,1 psi( pounds per square inch) was observed on the gauge for the 9x13 annulus vent and 7 psi( pounds per square inch) was observed on the gauge for the 5x9 annulus vent. At the time of this inspection a split gas sample with Moody and Associates was collected on all annulus vents. The 13x 20 was collected at 12:29 pm, the 9x 13 at 12:34 pm and the 5x9 was collected at 12:40 pm. No violations were noted at this inspection.
