Vegetative cover on site is greater than 70% with interspersed rocky areas. No evidence of accelerated erosion or sedimentation was observed.

A 2-year extension of time for site restoration was approved by the Department on March 07, 2016. The post construction stormwater management features approved as part of the extension request consist of an existing sediment basin and several rock lined swales. These features are in place and in good repair.

No violations were cited during this inspection.

Met with Jimmy Larsen at time of inspection. Well is closed in. A bridge plug was set near the surface on this well before drilling commenced on the 2 newer wells on the pad. 0% combustible gas was detected around the well at time of inspection. No violations were noted at time of inspection.

Met with Rockdale Marcellus representative Jimmy Larsen at time of inspection. Well was closed in. Cellar conditions did not appear to have changed from previous inspection. 0% combustible gas was detected around the well at time of inspection. No violations were noted at time of inspection.

2793379 Primary Facility 2018-10-23 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine plugging inspection of the Wesneski 724 1V was conducted October 23, 2018. At the time of inspection, a coil tubing rig is on location for final plug and abandonment. I spoke with the companyman to discuss operations on the pad and plugging procedure. No plugs have been set yet; they must first drill out existing retainer set in the 4 1/2" by previous operator. After drillout they will isolate perfs (6323'-6333) with CIBP and spot remaining cement plugs on CIBP and across intermediate and surface casing shoe depths (in 4 1/2") with dump bailer on WL. Gel spacers between plugs and gravel above surface casing plug to surface. 4 1/2" casing is reportedly cemented to surface and unrecoverable. There was reportedly no pressure outside of production casing. No issues were reported outside of production casing. No obvious issues were observed during this inspection.
2794369 Primary Facility 2018-10-24 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine plugging inspection of the Wesneski 724 1V was conducted October 24 2018. Coil tubing is off location after an attempt at final plug and abandonment. I spoke with the companyman to discuss operations on the pad since my site visit yesterday. The rig had difficulty drilling through an obstruction in the tophole section of the wellbore thought to have been a retainer but not listed on well records from previous operator. Plugging operations are shut down at this time, resumption of operations pending further investigation. At the well, cellar is full of water; no bubbling. Casing head is on 7" with (4 1/2" x 7") annular vent; no visible/audible venting or pressure felt. B section and master valve on production casing. No immediate issues were observed.
2803374 Primary Facility 2018-11-15 Administrative/File Review No Violations Noted Results were received for sample ID #0939077 taken on 10/17/2018 (see pictures). Sample was taken from a 15-20gallon puddle located outside of containment. Elevated barium, strontium, and chlorides indicate that the fluid on the pad was wastewater; the result of the administrative inspection is Violations Noted.
2824875 Primary Facility 2019-01-09 Routine/Complete Inspection No Violations Noted Light activity on the site when I arrived. 2 temporary sand separators currently hooked up to the recently fraced wells. The onsite modular storage structure was still present, but a worker was pumping water from the tank to a bank of Baker tanks in containment. The area around the MASS was dry, and a downgradient sediment trap nearby showed low conductance (.13 mS/cm). A truck loading area at the Baker tanks had a 10' x 5' (1-2" deep) in front of it that showed slightly elevated conductance; I recommend that the puddle be vacced. The area where I noted fluid with elevated conductance on my 10/17/18 inspection had recently had fresh gravel placed on it.
