2484561 Primary Facility 2016-05-03 Administrative/File Review No Violations Noted The Department of Environmental Protection (DEP) has reviewed the report titled “Final Closure Report, Vandergrift 290 Gas Migration Incident, Charleston Township, Tioga County, Pennsylvania, dated March 29, 2016”, and approved the document. The report was prepared by Moody and Associates, Inc. and submitted to the Department in accordance with the Oil and Gas Act, and constitutes a “Final Report” as defined in §78.89 (h) of the regulations. This report was reviewed by or under the supervision of a Pennsylvania licensed professional geologist. Based on water quality sample results to date, the DEP does not at this time anticipate requiring any further remedial action in relation to this matter. Although remediation is now complete for this site, you are advised that future issues may require additional investigation. The violations cited on 4/28/2011(Inspection ID 1970911) are considered resolved.

2829407 Primary Facility 2019-01-18 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Vandergrift 290 Well Pad in Charleston Twp, Tioga County on 1/18/2019 at 1240 hours. The weather was 32 degrees, cloudy and 1/4 inch of snow covered the pad surface.

This pad has been reduced in size and the resized portions are well vegetated. There are currently 6 wells (1H-6H) that appear to be capable of production with two production tanks. The site is well vegetated and remains stabilized. No erosional issues were observed today.

No violations or issues noted today, I departed the site at 1300 hours.
