2695407 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted Routine pre-plugging inspection of a vertical gas well. At the time of inspection: the well was shut in with nightcap bolted atop spool/hanger. There was a needle valve with an analog pressure gauge which read approximately 1600 PSI. The cellar was full of gravel with 4 annular vents coming up out of the gravel. No visual or audible flow was coming from the vents. I checked each vent with my hand; no pressure felt. No gas was detected with an Altair 5x gas meter at the vents.
2696963 Primary Facility 2018-02-22 Routine/Complete Inspection No Violations Noted Routine inspection of the Allen 620 IV attempted February 22, 2018. Plugging operations were tentatively scheduled to begin today according to Notice of Intent to Plug form submitted by the operator. At the time of attempted inspection; no crew or equipment was observed on the pad. A large truck was parked sideways across the lease road, obstructing access to the facility.
2722208 Primary Facility 2018-04-24 Routine/Complete Inspection No Violations Noted I arrived at the Allen 620 lease road on April 24, 2018 to conduct a routine plugging inspection. Operations were scheduled to begin today. The rig was pulling onto location when I arrived. I discussed the procedure briefly with a companyman and left, planning to return later this week once plugging operations begin.
2724109 Primary Facility 2018-04-26 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, plugging inspection of the Allen 620 1V vertical, unconventional well was conducted April 26, 2018. At the time of inspection, a rig is on location for plug and abandonment. Cementers are on location, circulating hole at the time of inspection with good returns. The pump operator shut down and I inspected all 4 annular vents (2 for backside of production casing where a pipe nipple and valve comes out of the 7" and 2 for backside of the intermediate presumably although the source of this vent was buried in cellar.) There was no visual or audible flow from the vents and no pressure felt. The pump operator mixed cement, came online and pumped bottom plug and displaced to balance with no issues and good returns. Cementers shut down and the rig pulled tubing out of the hole to spot the second half of the bottom plug. The well was dead and tubing pulled dry. They plan to finish the bottom plug, WOC overnight, tag, and pump tophole cement plugs tomorrow. No issues were observed.
2724153 Primary Facility 2018-04-27 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, plugging inspection of the Allen 620 1V was conducted April 27, 2018. At the time of inspection, a rig was on location for plug and abandonment. All cement plugs have been set at this time and the operator plans to cut and cap well tomorrow. I conducted a final gas check before the operator cuts, caps, and buries this plugged well. BOP was still nippled up, and the operator lined out 4 1/2" casing to side outlet on the BOP. No audible or visible flow of gas was observed coming from the well and no gas detected with an Altair 5x gas meter. No gas was observed or detected with a gas meter at the annular vents. No obvious issues were observed during this inspection.

2728604 Primary Facility 2018-05-10 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this plugged, vertical, unconventional well was conducted May 10, 2018. At the time of inspection; all equipment is off location following plug and abandonment. The well has been cut below grade and buried. There was no venting, bubbling, gurgling, or other obvious plugging-related issues observed at the surface location of the plugged well. No gas was detected with an Altair 5x gas meter at ground level above the plugged well.
