All E&S constrols remain in place, a record of weekly inspections on behalf of Seneca was located in the mailbox with E&S plan. The pad surface was partially stabilized, approximately 60% stone and 40% dirt. Several puddles were observed and tested; none exibited elevated conductivity. The slopes around the pad were partially vegetated, some were covered with mulch and some miror grass growth was observed. The establishment of the growth should be routinely observed. The cut slope across from the access should be stabilzed with perennial vegetaion capable of resisting accelerated erosion. THe 1V well was drilled and had awell tag on it. No new violations were cited during this inspection - MRN

2163708 Primary Facility 2013-05-01 Routine/Partial Inspection Violation(s) Noted A routine inspection of the DCNR 007 1V well site in Tioga County, Shippen Township was conducted on May 2, 2012. The last inspection documented on behalf of Seneca was listed as 4/3/2013. The pad surface was partially stabilized, approximately 60% stone and 40% dirt. Several puddles were observed and tested; none exhibited elevated conductivity. The slopes around the pad were vegetated in some area and bare soil in other areas.
2179586 Primary Facility 2013-07-01 Follow-up Inspection Outstanding Violations - No Viols Req'd A follow up inspection of the DCNR 007 1V well site in Tioga County, Shippen Township was conducted on July 1, 2013. Violations were cited in a 5/1/2012 inspection (2163708) for failure to maintain BMP failure to stabilize the site and failure to restore the site. The site has been temporarily stabilized with mulch, the routine inspections are being completed and documented in the mailbox. Violation of 102.22 and 102.4(b)(1) are considered resolved. The violation of 3216 of the 2012 Oil and Gas Act remain outstanding at this time. No new violation were cited during this inspection.
2296452 Primary Facility 2014-08-18 Compliance Evaluation No Violations Noted A site inspection of the DCNR 007 1V well site in Tioga County, Shippen Township was conducted on August 18, 2014. The site is stabilized with no erosion issues noted and no puddles or staining of the pad surface are evident at the time of this inspection. The well has been now plugged and the a monument indicated the plugging date as April 25, 2014. Violations cited in a 5/1/2012 inspection (2163708) for failure to restore are deemed corrected. To remain in compliance with Section 3216(d), this site must be restored within nine month after plugging the well. No new violations noted.
2480319 Primary Facility 2016-04-26 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the Seneca Resources – DCNR 007 Pad G well site (ESX10-117-0130 (01) on 4/26/2016 @1230 hours. The weather was 60° and cloudy with periods of rain showers. The winds were WSW @ 16mph and 69% humidity. The onsite E&S log for the DCNR 007 Pad G and Vert.50259 states the site was last inspected by company personnel on 4/18/2016. The boxes under site status for Production, Final Reclaimed, Signage, E&S plan, and E&S BMPs were initialed.

Seneca has placed a lock labeled “DEP” on the gate at the entrance to the site. Keys were provided to the Department to access the site. The violation of 3258(b) of 58 Pa.C.S §§3201-3274 (relating to Development) (“2012 Oil and Gas Act”) issued 4/8/2016 for not provided free and unrestricted access is considered resolved at this time. As of today’s inspection one well (API#117-20856) 5H was observed on the pad. A small poly tank appears to be connected to an annulus vent. Two constructed cellars were also observed. Two

2598065 Primary Facility 2017-05-18 Routine/Partial Inspection Violation(s) Noted On 5/18/17 at 1420 hrs I conducted a routine inspection of the DCNR 007 1V 50259 well pad. It was mostly sunny and 87 degrees F. No one was present during my inspection.

Construction of the 1V well pad was permitted under the ESCGP for the DCNR 007 Pad G site. The 1V well was drilled in 2010 and plugged on 5/25/14; a vent pipe and monument mark the plugged well. The well pad was restored in July 2015. The reclaimed area, which is fenced, is permanently stabilized.

On 2/29/16, Seneca submitted a Well Site Assessment Summary to the Department. The Summary milestone for the 1V 50259 well was to submit the Well Site Restoration Report (WSRR) in 2016. To date, the WSRR has not been received by the Department. Failure to submit the WSRR within 60 days of restoring the site is a violation of 25 Pa Code Section 78a.65(f).

The last routine E&S inspection was conducted on 5/12/17. I concluded the inspection at 1440 hrs. TLT

2731585 Primary Facility 2018-05-18 Site Restoration No Violations Noted The DEP conducted a restoration inspection of the Tract 007 Pad G 1V well (API#117-20640) located in Shippen Township, Tioga Co. on May 18th, 2018 @1015 hours. The weather was 62 degrees and sunny. Wind ENE 7mph 53% humidity.

There is no activity onsite. A monument with the well permit was observed at the well head. The well was plugged 4/25/2014. The site has greater than 70% vegetative cover. The fence has been erected around the perimeter.

No violations are noted at this time. I departed the site at 1051 hours.

2736279 Primary Facility 2018-05-31 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this plugged well was conducted May 31, 2018. At the time of inspection, the well has a 2" candy cane vent with marker plate erected as a plugged well monument. There were no visible or audible leaks observed, and no gas was detected at the vent. No obvious issues were observed.
