2369634 Primary Facility 2015-05-12 Routine/Complete Inspection No Violations Noted The Chicken Hawk site was inspected by the Department @1130 hours on May 12th, 2015. The weather was 68 degrees, partly cloudy with 68% humidity. 7 mph sustained winds out of the SW, with gusts up to 14 mph. The pad area was measured at 330'x381'. Containment, under piping leading to a temporary storage tank for fluid from the well head, appears to be breached and is no longer functional. This containment needs to be repaired. If this temporary containment is no longer in use, it needs to be removed from the site. Permanent containment surrounding the condenser was holding fluid with a conductivity reading of ~280µs/cm. Erosion blanketing being used as temporary stabilization, on the level spreader located off the eastern edge of the pad, has not achieved > 70% vegetation. Approval of this Notice of Termination is not recommended at this time. Please contact me with any questions.

2841999 Primary Facility 2019-02-19 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of Repsol Oil & Gas's Chicken Hawk Well pad in Sullivan Twp, Tioga County on 2/19/2019 at 1150 hours. The weather was sunny and 21 degrees. The pad surface is covered with snow and ice. This site was just recently acquired by Repsol from Tenaska Resources. The act 9 sign was observed at the entrance to the site. The onsite inspection log was found in the mailbox by the gate and was last inspected on 2/14/2019. The E&S Plan states the site was 20.9 acres.

There is currently 2 wells producing onsite (1H & 2H). The site remains well vegetated and stabilized. No releases were observed and containment structures appeared intact and functional. No PCSM BMPs are in place. No restoration extension or landowner consent agreement has been issued. An NOT was returned on 6/3/2015 for vegetation. No NOT or well site restoration report has been accepted.

The DEP recommends the current ESCGP be terminated and a new ESCGP that includes PCSM BMPs be obtained. Additionally, the DEP is
