2062329 Primary Facility 2012-04-25 Routine/Complete Inspection Pending The cut slope above the pad (below the frac tank pad) does not have perennial vegetation established. The growth should be monitored and re-seeded if necessary. A series of small puddles near the well exibited elevated readings for specific conductance (approximately 3-4 µS). A sample of the puddles and a sample of the surronding soils was collected and submitted for laboratory anlaysis. Violations are pending the results. James Wood was made aware of the situation via email at 1645 on the day of the inpsection. At that time it was recommended the soil be screened to determine the extent of the release. - MRN
2068978 Primary Facility 2012-05-18 Follow-up Inspection Violation(s) Noted Laboratory results for sample 0940-016 (the puddle) were received and indicated Barium was present in the liquid found on the pad surface at a concentration of 20,100 µg/L. This is approximately 10 times the primary standard MCL. Consequently, the Department’s inspections revealed violations of the Clean Streams Law, 35 P.S. § 691.1 et seq.; the Solid Waste Management Act, 35 P.S. § 6018.101 et seq; and Chapter 78.54.

During today's inspection the specific location of the puddle was identified and the approximate locations for test pits, the initial area to be excavated and areas where background samples could be collected were discussed. Material had been removed from the pad and stockpiled adjacent to the excavation and covered with a tarp. The violations cited during the 5/18/12 inspection (against the 1H well; 117-20532) remain outstanding at this time. No new violations were cited during this inspection.
2145685 Primary Facility 2013-03-07 Compliance Evaluation No Violations Noted On March 7, 2013 a Compliance Evaluation inspection was conducted at the Sylvester pad in Brookfield Township, Tioga County to follow up issues noted during a May 18, 2012 inspection (2068978). The impacted soil has been excavated and the area backfilled. Conductivity readings of several puddles on the pad were taken and none exhibited elevated readings. The violations cited during the May 18, 2012 inspection are considered resolved. No new violations were cited during this inspection. - MRN
2315793 Primary Facility 2014-10-15 Routine/Complete Inspection No Violations Noted The Department inspected the Tenaska, Inc. Sylvester well pad in Brookfield Twp., Tioga Co. on 10/15/14 @ 1140 hours. An adequate site ID sign was observed at the entrance. A well tag was observed on the Sylvester 1H well head, permit# 37-117-20532. The pad surface measured 280’ x 185’. Approximately 280’ x 80’ of the pad surface had been seeded and was growing vegetation. This area had not been regarded before being seeded. The cut slope contained ~60-70% vegetation. The fill slopes were fully vegetated and appeared stable. The cellar around the well head did contain some standing water. Puddles were observed on the pad surface and the conductivity was measured at 150 µs/cm. The containment around the produced fluid tanks contained standing water and the conductivity was measured at 750 µs/cm.
2380939 Primary Facility 2015-06-10 Site Restoration No Violations Noted An adequate site ID sign was observed at the entrance. A well tag was observed on the Sylvester 1H well head, permit# 37-117-20532. The pad surface measured 280’ x 185’. Approximately 280’ x 80’ of the pad surface had been seeded and was growing vegetation. This area had not been regraded to original contours before being seeded. The cut slope contained ~60-70% vegetation. Silt sock is still visible on the south side of the cut slope near the corner of the pad surface and the haul road. The fill slopes were fully vegetated and appeared stable. The PCSM/site restoration plan accompanying this NOT shows E&S controls are to be "reclaimed and restored to original control". At the time of this inspection, stilling basins/level spreaders are still visible but appear to contain ~70% vegetation. Silt sock is still visble along the haul road (see attached pictures).

Paul Torok, well tender for Victory Energy was onsite during my inspection. Mr. Torok last inspected the site on 12/31/2016 as noted in the onsite inspection log. A copy of the E&S plan was not available in mailbox. All E&S BMPs appear to be stabilized and functional. Cut and fill slopes appear to be stabilized with greater than 70% vegetative cover. Fluid was observed in the secondary containment surrounding the produced fluids tanks. The fluid conductivity was field measured at 126 microsiemens/cm, consistent with precipitation. 2813273 Primary Facility 2018-12-11 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of Tenaska Resource's Sylvester Well Pad in Brookfield Twp, Tioga County on 12/11/2018 at 1235 hours. The weather was partly cloudy, sunny and 28 degrees.

There is currently one producing well onsite with two production tanks. The pad is partially snow covered and fluids on the pad surface were frozen. Fluid within the GPU containment had a field measured conductivity of 200 us/cm today. The landowner is storing a few pieces of farming equipment onsite. No erosional issues were observed. The pad is very well vegetated and stabilized.

No issues or violations noted. I departed the site at 1300 hours.
