2064077 Primary Facility 2012-04-25 Routine/Complete Inspection No Violations Noted An inspection of the North Fork Pad was conducted at approximately 1300 hrs. on 4/25/12. The perimeter of the site was inspected. Multiple puddles were screened for elevated levels of conductivity and no elevated readings were found. The pad surfaces and surrounding slopes are stable. The culvert pipe associated with the access road and outlets near the gate in resulting in accelerated erosion. The erosion channel needs to be repaired and stabilized to avoid violations. A follow-up inspection will be conducted within two weeks to determine if the issue has been addressed. No violations were cited during this inspection. - MRN

2085087 Primary Facility 2012-07-18 Follow-up Inspection No Violations Noted A site visit was conducted on July 18, 2012 in response to issues noted in an April 25, 2012 inspection report (2064077). During this previous inspection it was noted that there was accelerated erosion evident below the sediment trap at the culvert pipe near the gate on the access road. During today's inspection it was found the issue was not yet corrected. This issue, as well as the fact that weekly inspections that are required under 25 Pa Code §102.4(b)5(x) were not completed since May 25, 2012 and that the site ID sign should be updated to reflect the current operator and emergency contact information, were brought to the attention of Bryan McConnell of Tenaska Inc. the day of this inspection. The issues were immediately addressed and documentation was provide to the Department to show this. (Filed in ESX10-117-0126 file). No violation were cited during this inspection. - MRN

2315924 Primary Facility 2014-10-15 Routine/Complete Inspection No Violations Noted The Department inspected the Tenaska Resources, LLC, North Fork well pad in Brookfield Twp., Tioga Co. on 10/15/14 @ 1315 hours. The weather was 66° rain with 93% humidity and wind gusts of 6mph. An adequate site ID sign was observed at the entrance. A well tag was observed on the North Fork 1H well head, permit# 37-117-20531. The pad surface measured 250’ x 250’. Approximately 250’ x 50’ of the pad surface had been seeded and was growing vegetation. This area was not re-graded before being seeded. The fill slopes were fully vegetated and appeared stable. Erosion control matting did not appear to be been installed correctly nor did it appear to be functioning. Please reference construction plan notes for installation instructions. According to onsite inspection logs the site was last inspected on 8/21/2014. The containment around the produced fluid tanks contained standing water, a field meter reading of the conductivity was 650 µs/cm. No violations were noted during the inspection. I departed the site @ 1345 hours.

The fill slopes were fully vegetated and appeared stable. The containment around the produced fluid tanks contained standing water, a field meter reading of the conductivity was 600 µs/cm.
2549106 Primary Facility 2017-01-04 Routine/Complete Inspection No Violations Noted Routine inspection conducted at the Tenaska Resources LLC - North Fork 1H well site located in Brookfield Twp, Tioga Co. The weather was partly sunny, 38 degrees, with 14mph winds. Snow/sleet squalls in the area. The gate at the access road of the site was unlocked and wide open when I arrived on site. At the time of this inspection two trucks from Waterworks labeled "residual waste" were draining the fluid from the secondary containment surrounding the produced fluid tanks. No other activity onsite. A portion of the pad surface appears to have been revegetated. One well appears drilled and in production on site. Cut and fill slopes appear stabilized with greater than 70% vegetative cover. No violations were cited during this inspection. Please contact me with any questions.
2813338 Primary Facility 2018-12-11 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of Tenaska Resource's North Fork Well Pad in Brookfield Twp, Tioga County on 12/11/2018 at 1310 hours. The weather was partly cloudy and 28 degrees.

The pad is well vegetated and stabilized. There is one producing well onsite (1H) with two production tanks. The landowner is storing a trailer at the entrance of the pad. No erosional issues were observed. Some snow cover was present and all fluids on the pad surface were frozen.

No violations noted today. I departed the site at 1340 hours.
