2066459 Primary Facility 2012-05-08 Routine/Complete Inspection No Violations Noted An inspection was conducted at the Austinburg 1H well pad on May 5, 2012. The site ID sign was present and E&S control plan was on-site. The last inspection conducted on behalf of Novus was on 5/7/12. No erosion issues were observed. The fill slopes of the pad are well vegetated and stabilized. The cut slopes are partially vegetated and may require additional seeding. Several puddles on the pad surface were monitored for elevated conductance. No elevated readings were found. The 1H well is the only permitted well on this site. Section 3216 of 58 Pa. C.S. §§ 3201 - 3274 (relating to Development) ("2012 Oil and Gas Act") requires an operator to restore the well site within 9 months after completion of drilling of a well. Consideration should be given to restoring the site or requesting an extension under the provisions of Section 3216(g). No new violations were cited during this inspection - MRN
2183058 Primary Facility 2013-07-10 Incident- Response to Accident or Event No Violations Noted On July 10, 2013 an inspection was conducted at the Austinburg 1H pad in Brookfield Township, Tioga County to monitor the monitor the corrective measures enacted by Tenaska Resources following a self-reported release of approximately 200 gallons of production fluid on May 14, 2013. A May 30, 2013 letter from Tenaska Resources (see file) outlines the remediation efforts to date. During this inspection the no fluids were observed on the pad surface. In a phone conversation with Bryan McConnell of Tenaska on July 12, 2013, he indicated the results of confirmatory samples (collected after the excavation of impacted soils was completed) would be submitted to the DEP. No violations were cited during this inspection.
2315985 Primary Facility 2014-10-15 Routine/Complete Inspection No Violations Noted The Department inspected the Tenaska, Inc. Austinburg well pad in Brookfield Twp., Tioga Co. on 10/15/14 @ 1400 hours. The weather was 66° rain with 93% humidity and wind gusts of 6mph. An adequate site ID sign was observed at the entrance. A well tag was observed on the Austinburg 1H well head, permit# 37-117-20530. The pad surface measured 300' x 300’. The cut slope contained ~70% vegetation. The fill slopes were fully vegetated and appeared stable. According to onsite inspection logs the site was last inspected on 4/1/2014. The containment around the produced fluid tanks contained standing water, the conductivity was measured @ 475 µs/cm. Erosion control matting below the rock lined ditch on the eastern side of the pad appears to be installed incorrectly and does not appear functional. Please consult the construction drawing notes for installation guidance. No violations were noted during the inspection. I departed the site @ 1430 hours.
2380126 Primary Facility 2015-06-10 Site Restoration No Violations Noted The PCSM/site restoration plan attached to the NOT shows the stilling basin/level spreader off the western edge of the pad was to be “reclaimed and restored to original control.” As of today’s inspection the restored still basin/level spreader area has not achieved 70% vegetative cover. Silt sock is still visible below the restored area (please see attached photos). Silt sock and stakes are visible along the access road as well as in the rock lined ditches. This site does not meet the requirements of §102.22(a)(1) regarding removal of temporary E&S BMP's. Old piping was observed stockpiled in the SE corner of the pad. According to Section 3216(c) of the 2012 Oil and Gas Act “The owner or operator shall remove all drilling supplies and equipment not needed for production
2549946 Primary Facility 2017-01-04 Routine/Complete Inspection No Violations Noted A routine inspection was conducted at the Tenaska Resources LLC - Austinburg 1H well site. The weather was 32? snowing with 21 mph winds. The gate at the entrance to the access road was unlocked and the access road was clear. Rock lined channels along the access road appeared to be functional. Bubbling was observed coming from what appeared to be an annulus vent in the cellar. The OGI inspector supervisor was notified of the findings for follow-up. Secondary containment surrounding the produced fluids tanks is more than halfway full and appears to be frozen solid. Pipe that does not appear necessary for production is stockpiled on site. Cut and fill slopes appear adequately vegetated. No violations were cited during this inspection. 2813260 Primary Facility 2018-12-11 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of Tenaska Resource's Austinburg Well Pad in Brookfield Twp, Tioga County on 12/11/2018 at 1120 hours. The weather was sunny and 27 degrees. Small amounts of snow covered some areas on the pad surface and topsoil piles.

There is currently one producing well onsite (1H) with two production tanks. The pad appears stabilized and well vegetated. Minor erosional channels were observed on the access road (see photo #4). Any fluids on the pad surface and within containment was frozen.

No violations noted at this time. I departed the site at 1150.
