2507283 Primary Facility 2016-08-03 Administrative/File Review No Violations Noted The Butler 127 Well Site was inspected in response to a Notice of Termination (NOT) and Well Site Restoration Reports (WSRR) received by the Department on June 29, 2016.

The well site has been partially restored. Areas not needed for production have been decompacted and vegetated. A total of 5,402.79 tons of drill cuttings, 1,749.21 barrels of drilling fluid, and 795.86 barrels of frack fluid were generated at the site and diposed of at approved landfill and recycling facilities.

The site was determined to meet the requirements of 25 Pa Code §102.22(a)(2)(i) Permanent Stabilization. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was observed. All temporary erosion and sedimentation BMPs were removed. No violations were cited during this inspection. The NOT was approved in Inspection Report No. 2505463.

The well site restoration reports are acknowledged.

2589357 Primary Facility 2017-04-18 Routine/Complete Inspection No Violations Noted On 4/18/17 at 1415 hrs I conducted a routine inspection at the Butler 127 well site. It was sunny and 60 degrees F. No one was present during my inspection. The 1H - 6H wells have been drilled and are in production; the 127D well has been placed on inactive status. The wells were tagged. The production equipment has built-in containment, which appeared to be in good condition. No staining or evidence of spills was noted on the pad surface.

Interim restoration has been completed and the site is permanently stabilized. Impervious areas have been minimized, temporary E&S controls have been removed and post-construction storm water management practices have been installed. The Notice of Termination and Well Site Restoration Reports were approved in 2016.

No violations were cited during this inspection. I concluded the inspection at 1545 hrs. TLT
