We arrived on site about 1300 hrs. Frac equipment was set up on location and operations were suspended pending investigation of the well control incident that occurred on Monday 1/17/11. We checked in with the TEUSA Completion Consultant, Sheridan Anderson. D. Zavislak tracked down those individuals that he needed to interview and proceeded to conduct the interviews, while I joined the 4-person TEUSA Investigation Team, led by Peter Shenkel - VP-D&C to inspect the valves and wellhead equipment involved in the incident. The most critical piece was the failed needle valve, which was recovered from the wellhead cellar.
