1944557 Primary Facility 2011-01-19 Drilling/Alteration No Violations Noted EP conducted an investigation inspection of the site on January 19, 2011 by Bruce Jankura, Oil & Gas inspector Supervisor and Darrell Zavislak, BOI Investigator. We arrived on site about 1300 hrs. Frac equipment was set up on location and operations were suspended pending investigation of the well control incident that occurred on Monday 1/17/11. We checked in with the TEUSA Completion Consultant, Sheridan Anderson. D. Zavislak tracked down those individuals that he needed to interview and proceeded to conduct the interviews, while I joined the 4-person TEUSA Investigation Team, led by Peter Shenkel - VP-D&C to inspect the valves and wellhead equipment involved in the incident. The most critical piece was the failed needle valve, which was recovered from the wellhead cellar.
