2652062 Primary Facility 2017-10-16 Routine/Complete Inspection No Violations Noted On 10/16/17 at 1453 hrs I conducted an inspection of the Willard 419 well site. It was mostly sunny and 56 degrees F. No one was present during the inspection.

The site was idle. Two wells, the 1H and 26H, are present at the site. The 1H was placed on inactive status on 1/16/14. Drilling of the 26H well commenced on 3/17/16 and the tophole portion was drilled. SWEPI submitted an application for inactive status for the 26H well in June 2017.

A gurgling noise was audible from a vent adjacent to the 26H well; this information will be passed on to the Oil and Gas Inspector for follow up, if necessary. The 22H and 24H wells were reported by SWEPI as "Proposed but Never Materialized".

The site consists of a well pad, water storage pad, access road and E&S controls. The site is permanently stabilized. SWEPI plans to conduct post-drilling restoration and installation of post-construction stormwater management practices during the upcoming 2018 construction season. The last routine E&S inspection was on 10/10/1

2681847 Primary Facility 2018-01-09 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP Willard 419 well pad located in Delmar Twp., Tioga Co. on 1/9/2017 @ 1245 hours. The weather was 35 degrees sunny. Winds WNW 15 mph. No one was present during my inspection. There was no activity onsite during my inspection.

The site consists of a well pad and water storage pad. According to a previous inspection report SWEPI intends to conduct post-drilling restoration and installation of PCSM BMPs during 2018. There are 2 wells on the pad the 1H and the 26H. The 1H was placeded on inactive status on 1/16/14. The tophole of the 26H well was drilled 3/17/16 but was placed on inactive status in June 2017.

The E&S inspection log shows the site was last inspected by company personnel on 12/27/17 with "no visible issues" recorded in the comments section. No issues were noted during the inspection. No violations are noted at this time. I departed the site @1400 hours.
