2048021 Primary Facility 2012-03-05 Routine/Complete Inspection No Violations Noted A routine inspection was conducted at the Hillside pad on Mar. 5, 2012. The 4H and 6H wells have been drilled, but are not yet in production and 5H well has a permit, but have not yet been drilled. During this inspection the ground was partially frozen. The perimeter of the site was walked and all erosion control BMPs were observed. There was no evidence of any spills found. Several areas, including the cut slopes of the access road, the fill slopes of the water impoundment and the cut slopes surrounding the pad are not stabilized. These areas must be covered with either a minimum uniform coverage of mulch and seed, with a density capable of resisting accelerated erosion and sedimentation or an acceptable BMP which minimizes accelerated erosion and sedimentation. A follow-up inspection will be conducted to determine if escalated enforcement is necessary. - MRN

2181351 Primary Facility 2013-07-03 Routine/Partial Inspection No Violations Noted On July 3, 2013 a routine partial inspection was conducted at the Hillside 804 pad in Gaines Township, Tioga County. The last inspection conducted by AMEC on behalf of the operator was on 7/2/2013. The permits associated with this site and their status area as follows: 117-20359 (1H - Operator Reported Not Drilled (ORND),117-20360 (2H - ORND),117-20361 (3H - ORND),117-20362 (4H - Inactive), 117-20363 (5H - ORND),117-20464 (6H - Inactive), 117-20948 (1H - ORND), 117-20949 (2H - ORND), 117-20950 (3H - ORND) and 117-20951 (5H - Active). Two wells have been spudded, but neither are in production. The fill slopes of the pad and impoundment are well vegetated and stable. The cut slopes are mostly vegetated but should be monitored until 70% perennial vegetation is established. Puddles on the pad surface were screened and none exhibited elevated conductance. No violations were cited during this inspection.
2465865 Primary Facility 2016-03-15 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Hillside 804 well site (ESG10-117-0019) located in Gaines Twp., Tioga Co. on 3/15/2016 @ 1145 hours. The weather was 45°, cloudy with 72% humidity, winds SW 2mph.
