Button 402 well pad (ESX09-117-0013) located in Jackson Twp., Tioga Co. on 2/22/2016 @ 1130 hours. The site appears to currently be in production. A single well was observed on the pad surface. Water surrounding the produced water tanks was frozen at the time of the inspection. A large amount of frozen water was observed in the RLD along the western edge of the pad surface. Cut and fill slopes appeared to be well vegetated and stable. RLDs along the access road appeared to be functional. No violations are noted at this time. The Notice of Termination for this site was approved 6/14/2014. Please contact me with any questions. I departed the site @ 1100 hours.

2854892 Primary Facility 2019-03-18 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of Repsol's Button 402 Well site in Jackson Twp, Tioga County on 3/18/2019 at 1330 hours. The weather was mostly cloudy and 36 degrees. The act 9 sign was observed at the site entrance. The E&S Plan was in the mailbox by the gate.

There is one producing well onsite (Button 402 1H). The site is well vegetated, stabilized and no erosional issues were observed. No PCSM BMPs are installed onsite. Containment structures were intact, functional and no leaks were observed. No evidence of spills or releases was on the pad. Puddles on the surface were field measured to be within normal conductivity ranges.

The two large strips of silt sock on both sides of the pad are no longer needed (see photos 4 & 5). These should be removed. No violations noted today. I departed the site at 1415 hours.
