Several attempts made to conduct an inspection, however, landowner grazing livestock around well pad and access road is gated. Site disturbance was less than 5 acres, therefore no ESCGP obtained and Notice of Termination not necessary. Erosion channel present from sump located beyond southeast pad corner. Channel extends from sump down through the landowner's pasture. Restoration Report will not be acknowledged at this time. A follow up inspection to be coordinated at a later date when conditions are more favorable to evalute the channel. TLT
2516089 Primary Facility 2016-09-02 Administrative/File Review No Violations Noted Administrative inspection following receipt of photos from SWEPI that document repair of an erosion channel off the SE corner of the restored well pad. Based on the documentation provided and the observations made during the 1/21/16 inspection, the Well Site Restoration Report is acknowledged. TLT
2808827 Primary Facility 2018-11-30 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (pre-plugging) inspection of this vertical unconventional well was conducted November 30, 2018. The well is shut in with night cap atop tubing hanger. Two wing valves on B section (no gauges). There was one open-ended annular vent observed; no audible or visible venting or discharge. The cellar was full of ice; no evidence of discharge or bubbling was observed. NOI has been received and plugging is tentatively scheduled for next week. There were no obvious integrity issues observed at surface during this inspection.
2810234 Primary Facility 2018-12-04 Plugging(Includes Plugged/Mined Through) No Violations Noted A workover rig is on location for final plug and abandonment. A crew is onsite putting second wing valve on B section, with hose, lined out to tank for returns. BOP is on location and about to be put on. I met with the Companyman to review and discuss the tentative procedure. They will run a gauge ring to bottom, set CIBP in 4 1/2", cap with cement with dumpbailer on WL, and proceed from there. 4 1/2" casing is cemented with an estimated TOC of 4784' and is believed to be recoverable. No pressures were reported. No obvious issues were observed during this inspection.
2811633 Primary Facility 2018-12-06 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this unconventional well was conducted on December 6, 2018. At the time of inspection, a workover rig is on location for final plug and abandonment. I met with the Companyman to discuss plugging operations. Since my last visit December 4, they have successfully set CIBP in production casing and spotted cement on top. 4 1/2" production casing has been successfully cut and recovered above the cement top. (TOC@ 4784', cut casing @ 4750'). The well is reportedly dead at this time. The next phase of the operation will be running back in with work string and spotting balanced cement plugs as planned. The crew intends to be rigged down and offsite early next week. The plugged well will be marked and buried. No obvious issues were observed or reported during this inspection.
2813576 Primary Facility 2018-12-11 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging, inspection of this unconventional well was conducted December 11, 2018. At the time of inspection, the rig and all equipment are off location following final plug and abandonment. The plugged well has been cut, marked, and buried below grade. There were no open holes, discharges, venting, bubbling, or any other obvious issues observed.
2840363 Primary Facility 2019-02-06 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of SWEPI's Hemenway TSRC 1 Well Site in Charleston Twp, Tioga County on 2/6/2019 at 1430 hours. The inspection was conducted in response to a post-plugging site restoration report received by the DEP. The landowner spoke with me onsite and mentioned that he is concerned about the casing downhole. I will follow up on this.

This site's access road and pad surface (0.45 acres) remain unreclaimed due to landowner agreement to keep in place. No PCSM BMPs are installed due to only 0.45 acres of land remaining. All equipment and materials related to the alteration of the well have been removed. The well was plugged and abandoned in December 2018. A well site restoration report has already been accepted by the DEP. The site remains stabilized and vegetated where applicable.

The post-plugging site restoration report has been accepted by the DEP. No violations noted. I departed the site at 1445 hours.
