2194956 Primary Facility 2013-08-12 Routine/Partial Inspection No Violations Noted On August 12, 2013 an inspection was conducted at the Marshlands H Bergey Unit 1 and Marshlands Play, Bergey 812 Pad in Gaines Township, Tioga County. The last inspection conducted on behalf of the operator was on 8/8/13, the only issue noted was that previously noted issues have not been addressed. During this inspection the following was noted: the cut slope above the pad is partially vegetated, but is in need of additional seeding in some areas. Much of the silt sock observed is in need of repair or removal. Further stabilization of the infiltration basin/berm to the southeast of the pad is needed. The site ID sign and permits posted should be reviewed to ensure the information on-site is accurate and up to date. No violations were cited during this inspection.

It was 28°, cloudy with snow flurries. 28 mph winds out of the west. An adequate site ID sign was located at the entrance to the access road. The mailbox was located on the back of site ID sign. The mailbox contained the onsite inspection log and a copy of the E&S plan. The site was last inspected by company personnel on 1/7/2016 with “no change” recorded in the comments section. On 12/22/2015 “areas of slumping on well pad cut slope monitoring” were recorded in the comments section.
2520232 Primary Facility 2016-09-14 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Bergey 812 site in Gaines Township, Tioga Co. on 9/14/2016 @ 1130 hours. It was 75°, sunny. An adequate site ID sign was located at the entrance to the access road. The mailbox was located on the back of site ID sign. The mailbox contained the onsite inspection log and a copy of the E&S plan. The site was last inspected by company personnel on 8/29/2016 with “no change” recorded in the comments section. On 8/24/2016 “pad vegetation coming in ~50%. Needs more veg on infiltration berms and ecm areas” were recorded in the comments section.

Two wells were observed onsite, the Bergey 1, API#117-20199, and the Bergey 812 3H, API#117-20498. Both wells had visible well tags. The pad size appears to have been reduced by revegetating a portion of the pad surface. No fluid was observed in the secondary containment surround the produced fluids tanks. 2621804 Primary Facility 2017-07-27 Site Restoration No Violations Noted On 7/27/17 at 1145 hrs I conducted an inspection of the Bergey 812 well site. No one was present during the inspection. It was mostly cloudy and 77 degrees F.

I observed the site (Photo #1) and reviewed the erosion and sedimentation (E&S) routine inspection log, the Notice of Termination (NOT) and the Well Site Restoration Report (WSRR).

The site consists of the access road, well pad and freshwater impoundment. Two wells, the Bergey 812 3H and the Marshlands H Bergey Unit 1 (Photo #2), have been drilled. According to DEP records, both wells are in production.

Produced water is stored in a production tank, which is staged in secondary containment (Photo #4). Standing water in containment had a field measured specific conductance of 455 micro Siemens/cm. Puddles on the pad were also tested; readings ranged between 280 - 415 micro Siemens/cm.

The impoundment is protected with a fence, had adequate freeboard and no holes were observed in the liner (Photo #3).

2791116 Primary Facility 2018-10-18 Routine/Complete Inspection No Violations Noted The DEP conducted a ROUTINE inspection of the SWEPI LP - Bergey 812 well pad, located in Gaines Twp, Tioga Co. on 10/18/2018 @1400 hours. The weather was 37 degrees, sunny, 65% humidity, winds SW @12mph. No activity was observed onsite.

The tanks and containment were last inspected by company personnel on 10/15/2018 with "tanks and containment were in good shape" recorded in the log. A slide that had occurred on the cut slope above the pad was observed stabilized by Erosion Control Blanket (ECB) and was growing ~25% vegetative cover. The freshwater impoundment was observed with adequate freeboard.

The Marshlands H Bergey Unit 1 and the Bergey 812 3H were observed in production. The pad surface has been revegetated and the site was NOT'd in July of 2017. No other areas of erosion or sedimentation were observed. No violations are noted at this time. I departed the site @1445 hours
