2850802 Primary Facility 2019-03-08 Routine/Complete Inspection No Violations Noted At time of inspection.

On site for a new spud inspection, accompanied by Eric Miller (OGI trainee for the Department). There were 8 wells on site with conductors drilled and set at 92'. Minichi Contracting Group was on site and drilled the 1H, 10H, 12H and 14H wells. United drilling was on site and drilled the 2H, 4H, 6H and 8H wells. The conductor pipe parted and became stuck in the hole of the 14H on 3/1/19, fishing attempts were unsuccessful. The operator contacted OGI supervisor Ken Kennedy on 3/4/19 and a plug and skid of 15' was approved. We took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

The purpose of this inspection was to ensure all perimeter BMPs were installed onsite before earth disturbance activities began the following day. Upon my arrival onsite, I was greeted by Mike Murnin (Cabot Oil and Gas-Construction Foreman).

Mr. Murnin stated that grading and tracking of the well pad site was in progress during the inspection. Construction of the nearby tank farm had just started; equipment had been mobilized the previous day and topsoil stripping was underway at the time of inspection.

Mr. Murnin also stated that the tank pad LOD and well pad LOD will eventually be joined, however the stream encroachment permit is still being reviewed. Until that permit is approved, the LODs remain separate. Furthermore, the plan indicates that a filter sock should be installed in the area of the stream encroachment where the two LODs will eventually join. However, since the encroachment permit has not yet bee
