2818858 Primary Facility 2018-12-18 Routine/Complete Inspection No Violations Noted I arrived on site at 1140 to conduct a routine complete inspection of the pad.

The pad was currently under construction, and the access road was only partially completed. Three excavators were being operated during the time of the inspection, all of them working in the vicinity of the pad surface itself (as opposed to the access road), and they appeared to be leveling out all the excavated material.

I noticed a section of riprap rock on the slope along the access road. Upon closer inspection, I observed running water flowing down the section of rock armor and into the rock-lined swale along the access road. A review of the plans on site indicated this section of riprap was added as a red line modification.

All E&S controls on site appeared to be in good condition and installed properly with the exception of one CFS section which was overtopping with water and had accumulated sediment that reached above the maintenance level (i.e. halfway point) of the sock's disturbed side. 2819741 Primary Facility 2019-03-28 Drilling/Alteration No Violations Noted I arrived at the site at 12:45. The weather conditions are partly cloudy skies with a moderate breeze, and temperatures around 50 degrees Fahrenheit. Precision Drilling Rig #181 Super Single is on the location. I met with Will Dean, well site supervisor, and consultant, and Mike Cromley, Environmental Health Consultant, both of Cabot Oil and Gas. Mr. Dean provided me with the following specifics relevant to site operations. "We are on the 2H well at a depth of (1,894) feet. The surface casing was cemented at (728) feet. The intermediate casing will be drilled to total depth (TD) at (2,120) feet. We will cement tonight. The well is being drilled with water based drilling mud. Currently there are no additives used. Sodium bicarbonate may be used as we get closer to TD of the intermediate casing. Clay/gel will get added in at TD. The rig has been here about (3) weeks. The rig will go to our M Powers pad after the work is done on the 2H well." I departed from the site at 14:10.
2844185 Primary Facility 2019-02-07 Preoperation Inspection No Violations Noted I arrived onsite at 1229 hours accompanied by Briana Cunningham to perform a pre-operational inspection of the pad. The weather was cloudy with moderate fog and a mild 45 degrees F.

During the inspection, there was a construction crew onsite removing large hay bails from a trailer; presumably for additional temporary stabilization as several of the slopes onsite containing erodible material had not yet been mulched and seeded.

After performing a site inspection and reviewing the redlined plan onsite, I agree that all perimeter E&S BMPs, namely CFS sections #1-#16, are installed properly and according to plan specifications.

2850695 Primary Facility 2019-03-08 Drilling/Alteration No Violations Noted At time of inspection.

On site for a new spud inspection, accompanied by Eric Miller (OGI trainee for the Department). There were 8 wells on site with conductors drilled and set at 118'. There were rig mats in place and drilling equipment being hauled to the well site. We took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
