2732874 Primary Facility 2018-05-18 Preoperation Inspection No Violations Noted The Department (Casy Baldwin and I) was on site to conduct a Preoperational Inspection in accordance with 3258(a.1). Upon arrival the Department reviewed the E&S Inspection Log which indicated that the most recent inspection was conducted on 5/17/18. The Department met with Mike Murnin (Construction Foreman-Cabot ) and Larry Cheatwood (Company Man-Cabot). This site was previously permitted under ESCGP ESX12-115-0191. The previous ESCGP was Terminated in May of 2016 and was re-permitted under the new ESCGP2 to enlarge the pad and drill an additional 9 wells. At the time of the inspection they were placing shot rock on the site. On the eastern side of the access road where it meets the pad they redlined a ditch on the fill and will grade it back and allow stormwater to travel to the previously existing access road roadside ditch.
