2720783 Primary Facility 2018-04-19 Incident- Response to Accident or Event Pending Arrived on location at: 13:30. Weather: Snow flurries, cold, ~ 30 degs. On location in response to a spill notification received from Cabot O&G Environmental Rep. Steve Watson. Steve called in notification to report a hydraulic oil spill of ~ 20 gals from a GDS Lowboy truck which occurred on the lower tank pad for the Mackey, R. Well pad.

- Upon my arrival to location I met with GDS HES Rep. Richard Allen and Resource Environmental Reps. Cory Peranich and Christian Mondi. Mr. Allen informed that a hydraulic line had burst on their Lowboy truck when loading a Dump truck on trailer. When leak occurred, GDS personnel deployed liner plastic, leak pans and absorbent pads to minimize ground impact. Mr. Allen stated that ~ 25 - 30 gals of hydraulic fluid had leaked out of tank but that most of that was captured. Mr. Peranich with Resource Enviro. stated that the initial PID screenings of spill location showed: 18.2 ppm at center of spill area and background check showed 0.1 ppm ~ 10' away. There was no visual signs

2760958 Primary Facility 2018-07-31 Routine/Complete Inspection No Violations Noted Today's inspection was conducted in response to Cabot's self reporting of a release of produced fluid directly to the COG Mackey R Tank Pad. On 7/30/18, Jon Sassi, Department EPS contacted Brody Webster, Senior Environmental Health and Safety Specialist of Cabot of Oil and Gas at 14:54 hours to inquire of the specifics of a reported spill of (2) barrels of water at the R Mackey site. Mr. Webster provided the following details in reference to the spill during that phone conversation. “A Susquehanna Gas Field Services Water Truck was in the process of delivering recycled water from Comtech to the Mackey Site to use it for completion of the wells. Mechanical failure of a pressure relief valve on the truck released an estimated (2) barrels of recycled water to the tank pad surface. The spill occurred at 1:45 AM on 7/30/18. I contacted the DEP after hours number at 2:04 AM. I spoke to the answering service and did not receive a return phone call. The area where the spill occurred , which is in front of the frac
2819639 Primary Facility 2019-04-01 Routine/Complete Inspection No Violations Noted I inspected the R Mackey well pad on April 1, 2019. The weather was above freezing, breezy and mostly sunny.

The access road and pad surface were stable and solid despite recent showers. The wells were properly labeled. There was a water hauler on site, as well as a Cabot welltender. There was also a Stallion trailer situated on the pad. No issues were noted.

The cut and fill slopes were all rocky and stable. The site was generally rock based, but wherever possible, vegetation was well established with no erosional issues noted.

There were no violations observed.
