2493839 Primary Facility 2016-06-08 Preoperation Inspection No Violations Noted On 6/8/16, the Department was on site at the COG Romeika, J. Pad 1 to conduct a Preoperational Inspection. The well permits that have been issued for this site are the Romeika 1H (115-22077), 2H (115-22078), 4H (115-22089), 6H (115-22090), and the 8H (115-22091). All perimeter BMP’s in place. Good temporary vegetative cover exists. E & S inspections up-to-date. No well drilling has occurred on the pad. Overall, the site looks good. No violations noted.
2524728 Primary Facility 2016-10-03 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 12:25p.m. for a new spud inspection, I met with company man James Cheek, United Drilling was on site to drill the holes and run the conductor pipe. Otex Pumping LLC was on site to cement the conductor pipe into place on the 8H and 6H at 149'. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 13:00p.m.

2585091 Primary Facility 2017-04-19 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 8:55a.m.

I met with company man Tony Ketterling. Nabors Rig X36 was on site and drilling the lateral section of the well. At time of inspection they were running in the hole with a reamer assembly to ream out the tight spots, they were at a depth of 6,644' with the reamer. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 8,732' Projected TD for the well is 18,200' I left the site at 10:15a.m.

2617081 Primary Facility 2017-07-18 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 12:45p.m.

I met with company men Larry Fulmer and Mike Vollmer. Keanes was on site to stimulate the 1H, 2H, 4H, 6H, and 8H wells. They were shut down to have a brace repaired on the frac missile. The 1H had 33 stages out of 51 completed. The 2H had 30 stages out of 41 completed. The 4H had 25 stages out of 48 completed. The 6H had 32 stages out of 65 completed. The 8H had 33 stages out of 69 completed. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:35p.m.

2643694 Primary Facility 2017-10-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently stimulated and put into production. The well tag is on the well and correct. All the annulus vent pipes are connected together and going to the same tank. The production fluid is going into a 500bbl flow back tank that is on containment and the containment appears to be in good condition. The 500bbl frac tanks are still on site and on containment. They are empty and the man holes are open. The containment appears to be in good condition. There were no obvious concerns noticed. There were no violations noted.

2682181 Primary Facility 2018-01-08 Routine/Complete Inspection No Violations Noted Today's inspection is being conducted as a routine inspection. All 5 wells, the 1H, 2H, 4H, 6H, and the 8H appear to be in production. Site appears to be stable. No spills detected. No violations noted.
2692638 Primary Facility 2018-02-08 Routine/Complete Inspection No Violations Noted Inspection conducted as a routine inspection. No E&S violations noted. No violations noted.
2723183 Primary Facility 2018-04-11 Incident- Response to Accident or Event No Violations Noted Gene Rickard, DEP O&G WQS and I arrived at the site at 11:14 in response to a reported spill of (5-7) gallons of production fluid to the pad surface. As we arrived there was no activity or personnel at the site. The weather conditions are mostly cloudy, cold and breezy with temperatures in the 30's. All wells on the pad are in production. We observed a Diaz container covered with a liner made of plastic material and a (55) gallon capacity drum labeled "Romeika 4/11/18 Brine Gravel GDS" on the pad surface near the production fluid and storm water tanks containment structure. In addition to observing the containers, we were able to determine that the location of the area on the pad surface remediated due to the spill was near the production fluid and storm water tanks containment structure. I conducted specific conductance (conductivity) screening of damp pad surface pad material just outside of the perimeter of the excavation of the impacted area near yellow sand bags, near the center of the excavation, storm
2726630 Primary Facility 2018-04-12 Follow-up Inspection No Violations Noted I arrived at the site at 12:57. Rain was falling with temperatures around 50 degrees Fahrenheit. Steve Catalfamo, Professional Geologist and Senior Environmental Health and Safety Specialist of Cabot Oil and Gas accompanied me during the inspection. The intent of the follow up inspection was to conduct field soil specific conductance (conductivity) screening in the general area on the pad surface where the production fluid spill occurred on 4/9/18. During an inspection at the site on 4/11/18, I recorded (3.02) millisiemens/cm conductivity in damp soil just outside the section of the pad surface remediated as a result of the spill. It had appeared as if the spill had not been adequately remediated. Conductivity was in the range of (0.5 -1.0) millisiemens/cm on 4/12/18 in the location where I recorded (3.02) milllisiemens/cm. The impacted area was backfilled with clean fill yesterday according to Mr. Catalfamo. Conductivity readings on my meter where less than (100) microsiemens/cm in the area backfilled. See
2739154 Primary Facility 2018-06-04 Administrative/File Review No Violations Noted This administration inspection report has been created to document a spill of an estimated (5-7) gallons of production fluid to the pad surface which occurred on 4/9/18 as documented in the Department's 4/11/18 and 4/12/18 inspection reports, Inspection ID #'s 2721248 and 2725656. Cabot submitted a Remediation Summary Report prepared by Resource Environmental of Montrose, PA to the Department on 5-23-18. The report contains a description of the incident, summary of remediation and characterization activities, waste disposal information, a site location map, a site map displaying the perimeter of the excavation, final soil conductivity screenings on 4/10/18, additional soil conductivity screenings on 4/13/18 and disposal documents of gravel impacted with produced fluids transported to Waste Recovery Solutions in Myerstown, PA.

The Description of the incident states the following information." At approximately 20:30 hours on April 9, 2018, a GasSearch Drilling Services Corporation (GDS) residual waste vacu

2749793 Primary Facility 2018-07-09 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 3' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2851338 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water and ice 5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 3/4/19. There was an empty 500bbl tank on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
