At the time of the inspection, the Site ID Sign had been installed and the E&S Plan was on site. The E&S Inspection Log indicated that a weekly inspection was conducted by Dillon Otten on 1/12/15. The E&S plan had several redlines including additional sections of filter sock added, two elongated soil stockpiles, a fuel containment area, and a temporary culvert.

I took a walk around the location and everything looked OK.

I left the site at 15:00p.m.

At the time of my arrival, I met with Ray Peplinski (Chief Inspector-Williams). The E&S Inspection Log stated that the most recent inspection was a post precipitation inspection conducted on. 5/15/17 by Anna Bonczek (TEC).

The cellar was full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 581psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:30p.m.

The well was in production. There was ice at the bottom of the cellar and there was no gas detected. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2806035 Primary Facility 2018-11-21 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There cellar was full of water and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 11/2/18. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
