2353795 Primary Facility 2015-03-12 Routine/Complete Inspection No Violations Noted Precison Drilling Rig #569 is currently in the process of drilling the N Powers 8H well at a depth of (13,036.71) feet. I observed pockets/spots of a substance yellowish-greenish in color where I parked my truck on the pad surface next to the site perimeter earthen berm and cut slope at N-41°50’ 40”; W-76° 01” 86”. Mr. Cormier claimed this substance that has been released to the pad surface may be either bio-lube or glycol. The amount of this material released to the pad surface did not appear to be significant and is isolated to a small area. Mr. Cormier informed me that he would have the material removed from the pad surface with a shovel and a bucket immediately.

2409517 Primary Facility 2015-09-22 Follow-up Inspection No Violations Noted Remarks: At time of inspection, arrived on site at 8:45 a.m. for a 72hr PBUT, accompanied by DEP Geologist Eric Rooney. I met with Cabot representatives Arlen Greig and Brian Davis and Selman representatives Brett Wanbold, Tyler Thomas and Prad Jadeja. Eric used an Altar 5X gas detector calibrated on 9/21/15 to check for gas show in all the annuli’s. Selman reps used a Gascope model 60 gas indicator and a Sensit HXG-3 gas detector to confirm gas show was present. If gas was detected, then Eric Rooney and Brett Wanbold took gas samples. Wells were shut in on 9/18/15 at 12:00 p.m. We started taking pressure readings at 9:25 a.m. with a Crystal pressure gauge supplied by Cabot reps. and checking for gas show at 10:10 a.m.
2451721 Primary Facility 2016-02-22 Follow-up Inspection No Violations Noted At time of inspection, arrived on site at 13:20p.m. for a 72hr PBUT, accompanied by DEP Geologist Eric Rooney. I met with Cabot representatives Shawn Christ, Adam Vogt and Larry Fulmer, and Selman representatives Mariah Palmatier and Prad Jadeja. The 2H and 3H wells were shut in on 2/19/16 at 11:56 a.m. by Adam Vogt. Pressures were taken from a Wika pressure gauge that was calibrated on 2/19/16 and Eric used an Altar 5X gas detector calibrated on 2/22/16 to check for gas show in all the annuli’s. We started taking pressure readings at 13:45 p.m., when gas was detected it was confirmed by Prad Jadeja.

Results of gas shows are on page two. I left the site at 14:10p.m.

2451898 Primary Facility 2016-01-21 Routine/Complete Inspection No Violations Noted The Department was on site on 1/21/2016 at the Powers N Pad 1H, 2H, 3H, 5H, and 8H (ESX29-115-14-0001) to conduct a Routine Complete Inspection. Well Permits issued for this pad are the 115-21906,21907,21908 and 21909.

The site was vegetated and there were currently no erosional issues. The vegetation looked healthy. There was a tank battery on site, and the site was stable. The site's last E&S inspection was on 1/19/2016.

There were no violations.

This qualifies as the preoperational inspection.

2454610 Primary Facility 2016-03-01 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:25 a.m., Pumpco Services was on site fracking the wells. I met with company man Larry Fulmer, they were fracking the seventh stage on the 8H, there are 7 stages completed on the 2H and 5H, and 4 stages completed on the 3H. We took a walk around the location and everything looked OK.

I checked for free gas at the water well at 11:00 a.m. and there was no gas detected. I left the site at 12:10 p.m.

2487366 Primary Facility 2016-05-12 Routine/Complete Inspection No Violations Noted I conducted an inspection of the site accompanied by Briana Cunningham, PADEP O&G WQS. Cabot submitted stimulation notices to the Department on 2/16/16 indicating that hydraulic fracturing operations for the N Powers 2H, 3H, 5H and 8H wells would commence on 2/25/16. These wells are close to being in the production phase of operations. Frac tanks used during well completions operations remain on-site. The tanks are stored in a tank battery on containment in the Northwest corner of the pad at the base of the cut slope. The containment berms for the tanks have been removed. There was quite a bit of water accumulated on the containment, most likely due to the recent rainfall events. I recorded (4.42) milliSiemens/cm³ conductivity in water on containment behind the blue frac tanks near the site perimeter storm water ditch and near the base of the cut slope, and (6) ms/cm³ conductivity in between (2) rows of green frac tanks near the center of the tank battery. 2503757 Primary Facility 2016-07-21 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:30a.m., the wells are in production with the production line connected to a sand separator and all fluid going into a gas buster flow back tank that is on containment and the containment is in good condition. The cellar was full of water with no bubbling noticed, the (13x9) vent pipe was open and venting into a tank, the (9x5) vent pipe was open and venting into a tank, there was 515psi on the production casing, I took a walk around the location and everything looked OK.

I left the site at 12:05a.m.

2515457 Primary Facility 2016-08-23 Incident- Response to Accident or Event No Violations Noted Frank Hoehle, DEP O&G WQS, and I arrived at the site at 10:45 to investigate the discovery of an additional area of the tank pad surface impacted due to a release. The Department conducted a Routine Complete Inspection of the site on 6/27/16. During that inspection, the Department discovered an area of staining white in color, and crystallization in the Northeast corner of the tank pad where a tank farm was located. The Department poured organic free reagent water on the dry surface in order to obtain conductivity readings in this particular area. A Hanna HI 98331 Soil Test EC & Temp meter was used to record conductivity readings. The readings were in the range of (600-720) microSiemens/cm³. The tank farm consisted of frac tanks staged on secondary containment. Mr. Watson contacted the Department on 7/27/16 upon receipt of the Department's 6/27/16 inspection report. The cellar had water at the bottom with no bubbling noticed. The (20x13) annulus vent pipe was open and venting into a small plastic tank, the (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 572psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:25p.m.

2703571 Primary Facility 2018-03-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. I took a walk around the location and there were no obvious concerns noticed.

2805370 Primary Facility 2018-11-20 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 7.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 11/2/18. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
