2502186 Primary Facility 2016-07-15 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:05p.m., the cellar was full of water with no bubbling noticed, the (13x9) vent pipe was open and venting to a tank, the (9x5) vent pipe was open and venting to a tank, there was 446psi on the production casing, I took a walk around the location and everything looked OK.
I left the site at 13:40p.m.
2547818 Primary Facility 2016-11-30 Routine/Complete Inspection No Violations Noted The Department was on site to conduct a Routine Complete inspection of the Powers N Pad 2 (ESX13-115-0067).

The wells associated with this site are as follows: 115-21777 115-21778 115-21779 115-21780

There was no activity on the site, and while some areas of growth were not excellent, there was sufficient growth occurring to ensure site stability. There did not appear to be any sediment leaving the site at this time, and the area was stable.

2593757 Primary Facility 2017-05-09 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 14:00p.m.

The cellar was full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 516psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:35p.m.

2597007 Primary Facility 2017-05-24 Routine/Complete Inspection No Violations Noted The site was well constructed and stable. There were no erosional events occurring at time of inspection, and the vegetation on site was mostly adequate, except where it seemed to be struggling a bit on the cut slope. There was no erosion in this area however. The pad surface was stable. There was no bubbling in the well cellars.

The well was in production. The cellar was full of water and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and venting to the same tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2717131 Primary Facility 2018-04-10 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Powers N 2 well site. The site was well vegetated on the cut and fill slopes. The pad surface was solid as was the entrance. The wells were properly labeled. There were no erosional issues apparent. The wells associated with this site are the 115-21777, 115-21778, 115-21779 and 115-21780.

There were no violations.

2733969 Primary Facility 2018-05-23 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Powers N Well Site. The wells associate with this site are the 115-21777, 115-21778, 115-21779 and 115-21780. The wells were properly labeled.

The pad surface was stable and had quite a bit of standing water on it due to recent heavy rains.

The cut and fill slopes were well vegetated and had no erosional issues apparent.

There were no violations noted.

2787186 Primary Facility 2018-10-09 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Powers N2 Well site.

The weather was overcast and in the low 70s.

The short access road was stable, as was the pad surface. The wells were all properly labeled.

The cut and fill slopes were all well vegetated, in excess of the 70% perennial vegetative standard required by the Department. The slopes were well graded and stable.

The PCSM infiltration basin was well constructed and well vegetated.

The site appeared to match the plans provided to the Department.

There were no erosional issues noted.

There were no violations.

2798434 Primary Facility 2018-11-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water .5' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 11/2/18. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
