2275284 Primary Facility 2014-05-29 Routine/Complete Inspection No Violations Noted with the pads not staked out and with the LOD not marked, the Department is unable to make a complete evaluation of the site in relation to the identified wetlands. Please stake out the pads and mark off the LOD near the vicinity of the identified wetlands. Also during the inspection, wetland markers were located within what appeared to be the well pad location. Mr. Thompson stated that he did not know why the wetland markers were placed in the well pad vicinity, but suspects that they may have been placed by consultants for the Williams gas pipeline. Please submit a report to the Department explaining why there is a discrepancy in the delineations identified by Cabot and Williams and what the final determination of the wetland delineations is. Once the above mentioned issues are completed, the Department will re-inspect the site.
2276376 Primary Facility 2014-06-04 Routine/Complete Inspection No Violations Noted Inspection conducted to evaluate the siting of a proposed well pad and tank pad in relation to a delineated wetland greater than 1 acre. The inspection is based on Cabot’s submittal of an OG 57 waiver request for their proposed Weiss M . Well Site and Tank Farm. The distance to the wetland is as described. The proposed additional measures described in the application should be adequate in protecting this resource. This field inspection supports the approval of the waiver request. Also present during the inspection was Andrea Mullen, Department Compliance Specialist.
2294139 Primary Facility 2014-08-07 Preoperation Inspection No Violations Noted The Department was on site at the Cabot Weiss, M. Well Site and Tank Farm to conduct a Preoperational Inspection. Well permits that have been issued for this pad are the 1H (115-21746), 2H (115-21747), and the 3H (115-21748). All perimeter BMP’s in place to include red lined additions. The well pad is approximately 95% complete. The tank pad is approximately 75% complete. No violations noted.

2504039 Primary Facility 2016-07-21 Routine/Complete Inspection No Violations Noted On 7/21/2016, the Department conducted an inspection of the COG Weiss M Well Site. Two of the three wells permitted for this site have been drilled, stimulated, and appear to be in production, the 1H and the 2H. The 3H has not been drilled. Both the well pad and the tank pad are stable and have very good vegetative growth. There was a minor amount of standing water on the well pad near the well heads. A field conductivity test of this water revealed the conductivity to be around 500 milliSiemen/cm³.

2562347 Primary Facility 2017-02-14 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 15:10p.m., the cellar was full of water and ice with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into a small plastic tank, the (9x5) annulus vent pipe was open and venting into a small plastic tank. There was 602psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 15:40p.m.

2652252 Primary Facility 2017-10-25 Routine/Complete Inspection No Violations Noted At time of inspection.

DLH Energy Service was rigging up a swabbing rig on the 1H. The containment appeared to be in good condition. There was a 500bbl flow back on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2751547 Primary Facility 2018-07-12 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 4' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2839784 Primary Facility 2019-02-13 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. As the photo's show, the site is snow covered. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
2851590 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 2.5' below grade in the cellar with water running in from the snow melting on the well pad and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 3/4/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
