I took a walk around the location and everything looked OK.

I left the site at 11:30 a.m.

The (13x9) vent pipe was open and venting into a small plastic tank, the (9x5) vent pipe was open and venting into a small plastic tank. There was 525psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 10:05a.m.

2632035 Primary Facility 2017-09-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. I took a walk around the location and there were no obvious concerns noticed.

2657016 Primary Facility 2017-11-06 Routine/Complete Inspection No Violations Noted I inspected the Millard K P1 well site on November 2, 2017. The site had a long access road which was stable and solid. The site's cut and fill slopes were as well vegetated as possible considering the extent of rocky ground present on site. There were no stability or erosional problems apparent.

There were no violations noted.

2728890 Primary Facility 2018-05-11 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection of the Millard well site. The wells associated with this site are as follows: 115-21686, 115-21689. The wells were properly labeled.

The access road and pad surface were firm and stable. The slopes along the access road were stable.

The cut slope was very rocky as was the fill slope. Where possible, vegetation was growing but the majority of the surfaces was comprised of rock, so little vegetative growth was possible. The slopes were solid and stable. There were no erosional issues observed during this pad inspection.

There were no violations noted.

2741615 Primary Facility 2018-06-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2836450 Primary Facility 2019-02-06 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water and ice 2.5' below grade in the cellar and there was no bubbling noticed. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 2/5/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
